腔镜甲状腺切除术中的风险因素分析及护理

摘　　要：目的探讨腔镜甲状腺切除术中的风险因素,并总结护理要点。方法对本院收治的178例患者实施甲状腺切除术,分析术中风险因素,并做好术中护理工作。结果患者均顺利完成手术,手术时间60~100 min,平均（75.02±18.21）min;术中5例患者发生皮下气肿,1例发生血管损伤出血,6例发生喉上、喉返神经损伤,1例发生气管损伤,及时采取有效措施,均得到恢复;术后随访3~24个月,均无复发。结论本组腔镜甲状腺切除术患者术中风险包括皮下气肿,喉上、喉返神经损伤,气管损伤、食管损伤,术野不清晰等,根据原因采取有效护理措施,确保了手术顺利进行。Objective To explore the risk factors during endoscopic thyroidectomy（ET） and summarize the related nursing measures. Method Data of 178 patients undergoing ET was analyzed to find risk factors related to ET and summarize the nursing measures. Results The ET procedures were all successfully performed with an operation time of 60 ~ 100 min and an averaged time of75.02 ± 18.21 min. Five patients affected with subcutaneous emphysema, one with vascular injury during surgery, 6 with nerve injury and one with trachea rupture. Conclusions The risk factors of the procedure include aerodermectasia, recurrent and superior laryngeal nerve injury, intraoperative bleeding and unclear operative field. The corresponding nursing interventions can help the successful accomplishment of the operation.
