压疮联合蚕食清创法的临床效果观察

摘　　要：目的：探讨压疮联合清创法的效果。方法：按入选标准选择入住我院神经内科54例74处Ⅱ-Ⅳ期压疮患者，按照入院顺序随机分为三组各18例，即自溶清创组（24处）、单纯锐器清创组（24处）和联合蚕食清创组（26处），入院后按操作流程进行压疮评估和清创，清创后均给予美清佳涂抹、保湿敷料等覆盖至愈合。记录三组的压疮愈合计分、黑黄红期演进需要的清创时间，使用单因素方差分析比较三组的结果。结果：自溶清创、单纯锐器清创和联合蚕食法清创三种清创方法的清创期时间分别为（34．29±17．99）d、（20．99±10．31）d和（17．81±8．56）d，差异有统计学意义（P〈0．01），联合清创法的清创期时间短于其他两种方法的清创期时间。结论：联合清创法采用清创胶将坏死组织软化水解后分次剪除，安全有效，缩短清创期，是Ⅱ一Ⅳ期压疮患者理想的清创方法。Objective Discuss the effect of pressure sores in combination with nibble debridement method.Methods Choose our hospital neurology 54 cases （74） Ⅱ-Ⅳ period in patients with pressure ulcers, were randomly divided into 3 groups, all 18 cases. Autolyzed debridement group （24）, pure sharps debridement group （24）, joint nibble debridement group （26）. Record score of the three groups, for pressure ulcer healing. the debridement time that from black to yellow, analyzing and comparing the results of the three groups.Results Autolyzed debridement group, pure sharps debridement group, joint nibble debridement group, of debridement time respectively （34.29 ± 17.99） d, （20.99±10.31） d and （17.81±8.56） d. Method of joint nibble debridement period time shorter than the other two methods. The difference was statistically significant （P 〈 0.01）.Conclusions Joint nibble debridement method is safe and effec^e, can shorten the debridement, is ideal methods ofdebridement for patients with Ⅱ-Ⅳ period pressure ulcers.
