健心颗粒及拆方对心力衰竭大鼠细胞凋亡的影响

摘　　要：目的:探讨健心颗粒及拆方对慢性心力衰竭(CHF)心肌细胞凋亡的影响,为分析此方中医药组方原理和临床应用提供实验依据。方法:以老龄雄性自发性高血压大鼠(SHR)的CHF模型作为研究对象,根据中医治法将健心颗粒拆分为益气组、活血组、温阳组和利水组,观察全方及各拆方组对CHF大鼠左室压力上升和下降最大速率(±dp/dtmax)、血管紧张素Ⅱ和凋亡蛋白Bax、Bcl-2表达的影响。结果:健心颗粒全方及各拆方组均可以改善CHF大鼠心功能和下调Bax蛋白表达,上调Bcl-2蛋白表达(P<0.05或P<0.01),全方组优于各拆方组;各拆方组间比较,益气组、利水组改善±dp/dtmax、抑制AngⅡ作用优于温阳组、活血组(P<0.05);益气组、活血组下调Bax、上调Bcl-2蛋白表达作用优于温阳组、利水组(P<0.05)。结论:在改善心功能、抑制AngⅡ水平和抑制心肌细胞凋亡方面,健心颗粒各拆方组作用略有侧重,而健心颗粒全方作用最全面、显著,说明益气、温阳、活血、利水四种治法配伍有协同作用。Abstract：Objective ：To explore the effect of fianxin granules and its decomposed recipes on myocardial cell apoptosis due to chronic heart failure（ CHF）, so as to provide the experimental basis for analysis of TCM principles in the prescription and clinical application. Methods： CHF models of older male spontaneously hypertensive rats （SHR） were taken as researched object. According to the principles and treatments of TCM, fianxin granules was decomposed into yiqi, huoxue, wenyang, and Lishui groups. The effects of fianxin granules and its various decomposed recipes groups on rising and falling maximum rates （＋ dp/dtmax） of left ventricular pressure, angiotension-Ⅱ（Ang-Ⅱ） and expressions of apoptotic proteins Bax, Bcl-2 in rats with CHF were observed. Results： The whole recipe offianxin granules and its various decomposed recipes groups could all improve cardiac function of CHF rats,reduce the expression of Bax protein and Ang- Ⅱ and increase the expression of Bcl-2 protein （ P 〈 0.05 or P 〈0.01 ）. The whole recipe was better than its various decomposed recipes groups. Compared among the various decomposed recipes groups, the effects of improving ± dp/dtmax and inhibiting Ang-Ⅱ in Yiqi and Lishui groups were better than in wenyang and huoxue groups （P 〈 0.05）. The effects of reducing the expression of Bax protein and increasing the expression of Bcl-2 protein in yiqi and huoxue groups were better than in wenyang and Lishui groups（P 〈0. 05）. Conclusion ： In improving cardiac function and inhibiting Ang Ⅱlevel and myocardia cell apoptosis, the roles of yiqi ,wenyang, huoxue and lishui drugs from fianxin granules have respectively different emphases, but the effect of whole recipe of fianxin granules is the most comprehensive and significant, which shows that the compatibility of four therapies of yiqi,wenyang, huoxue and lishui has a synergistic role.
