摘 要:目的探讨声触诊组织成像(virtual touch tissue imaging , VTI)技术在诊断BI—RADS4级乳腺病变中的应用价值。方法应用VTI技术对68例患者76个经常规超声诊断为BI—RADS4级的乳腺病灶进行检查,获得并分析VTI弹性图,对病灶硬度进行评分,计算病灶的VTI弹性图与灰阶图面积比。绘制ROC曲线,找出VTI评分、VTI弹性图与灰阶图面积比诊断乳腺BI—RADS4级病变良恶性的最佳截点。结果VTI硬度评分法诊断BI—RADS4级乳腺恶性病变的最佳截点为4分,敏感度为88.6%、特异度为82.9%;VTI图与灰阶图面积比的最佳诊断截点为1.85,其敏感度为80.0%、特异度为85.4%。结论VTI技术对BI—RADS4级乳腺病变良恶性诊断有较高的应用价值。Objective To assess the application of virtual touch tissue imaging (VTI) in the diagnosis of BI - RADS 4 breast lesions. Methods VTI was performed on 68 patients with 76 breast lesions in total which were classified as BI - RADS 4 by conventional ultrasonography. The images obtained underwent analysis. The stiffness of masses was scored, while the ratio of areas of VTI images to gray -scale images were calculated. Moreover, using pathological diagnosis, a receiver operating characteristic (ROC) curve was drawn to determine the optimized cut - off point predicting whether BI - RADS 4 breast cancer was benign or malignant based on VTI scores and the ratio of areas of VTI images to gray - scale images. Results According to the stiffness of breast cancers, the optimized cut - off point was set at 4 for BIRADS 4 malignancy lesions ,with a sensitivity of 88.6% and a specificity of 82.9%. Meanwhile, the ratio of areas of VTI images to gray - scale images had the optimized cut - off point of 1.85, with a sensitivity of 80.0% and a specificity of 85.4%. Conclusion VTI is applicable to determine whether BI - RADS 4 breast cancer is benign or malignant.
