摘 要:目的有效落实卫生行业新标准《护理分级》,进行持续质量改进。方法通过团队的组建、护理分级质量现状调查、根本原因分析和验证、改进方案的制订和实施、有效质量控制等干预措施进行护理分级质量改进,采用护理分级合格率、患者满意度和医护人员看法来进行效果评价。结果改进后护理分级合格率高于改进前,差异有统计学意义(χ2=67.066,P<0.001);患者满意度中"业务水平""健康教育""满足需要的及时性"高于改进前,差异有统计学意义(t=-2.318,P=0.022;t=-4.929,P<0.001;t=-3.528,P=0.001);访谈医护人员均对质量改进活动给予肯定,表示改进后的护理级别更符合患者需求。结论优化护理分级流程,采用看板管理进行质量控制,科学进行自理能力分级,是落实《护理分级》标准、提高护理分级合格率和患者满意度的关键。Objective To continuously improve the quality of patient classification and effectively implement the standards of patient classification system. Methods The quality of patient classification was improved through team building,baseline assessment,analysis and verification of root causes,formulation and implementation of improvement measures and quality control. The outcomes were evaluated using the qualified rate of patient classification,patient satisfaction and appraisal from doctors and nurses. Results The qualified rate of patient classification increased from 73.47% to 93.31% (x^2=67.066,P〈0.001). Patient satisfaction with nurses' professional level,health education and service timeliness were improved (t=-2.318,P=-0.022;t=-4.929,P〈0.001 ;t=-3.528,P=0.001). Doctors and nurses reported that the improved patient classification system could better meet patients' needs. Conclusion Optimizing the process of patient classification,the use of Kanban management for quality control and classification of self-care ability are key points to implement patient classification standard,improve the qualified rate of patient classification and increase patient satisfaction.
