摘 要:目的:为医院创建用药咨询中心的药学服务模式提供参考。方法:分别从咨询药师的职业要求、咨询工作依据、内容与模式和咨询用相关辅助设施设置等方面阐述用药咨询中心的创建过程。结果:咨询药师经过统一培训与考核管理,配备专用的咨询软件、书籍以及设立药学服务标识、教具、引导标牌等设施作为保障,除可为一般性患者用药提供指导外,还可为特殊患者提供特色咨询,并开展药师延伸服务等,形成用药咨询服务的标准化和规范化模式。结论:医院创建用药咨询中心,可提升公众的安全用药意识,改善患者的用药依从性,凸显药师的职业形象,提高药学服务品质。OBJECTIVE: To provide reference for the establishment of pharmacy service mode in the medication consultation center of hospital. METHODS: The establishment process of the medication consultation center was described in aspects of the oc- cupational requirements for consultant pharmacists, basis, contents and mode of consultation work and the setting of auxiliary facili- ties for consultation. RESULTS: A standardized and normalized mode of medication consultation services may be formed by train- ing and conducting appraisal management over consultant pharmacists in an unified manner, providing the software dedicated to consultation, books and facilities such as pharmacy service signs, teaching aids and guide signs, as the guarantee of consultation work, offering special consultation for special patients in addition to guidance for general patients, and pharmacists' rendering ex- tended services. CONCLUSIONS: The establishment of the medication consultation center in hospital may improve the public's awareness of medication safety, promote patients' compliance, highlight pharmacists' professional images and improve the quality of pharmacy services.
