难治性糖尿病性胃轻瘫20例临床研究

摘　　要：目的研究α-硫辛酸联合香砂六君子汤对难治性糖尿病性胃轻瘫(Diabetic Gastroparesis,DGP)的治疗效果。方法:将甲钴胺及莫沙必利联合治疗效果不佳的DGP患者20例,随机分为观察组和对照组。对照组继续甲钴胺及莫沙必利联合治疗,观察组在对照组原治疗方案基础上加用α-硫辛酸联合香砂六君子汤治疗,疗程均为2周,两组均予以积极生活方式干预和胰岛素良好控制血糖,观察两组治疗前后临床有效率、血清胃动素(motilin.MTL)水平及肝肾功能变化。结果:治疗2周后两组临床疗效比较差异有统计学意义(P<0.05),观察组好于对照组。两组血清MTL水平均有下降(P<0.05),且观察组下降值较对照组更明显(P<0.05)。结论:α-硫辛酸联合香砂六君子汤治疗难治性DGP临床有效率高,安全且耐受性好,可能与α-硫辛酸强力抗氧化应激,改善神经功能缺损和香砂六君子汤抑制胃动素分泌,加快胃肠道运动相关。Objective： To study the treatment effect of α- lipoic acid combined Xiangsha Liujunzi on refractory diabetic gastroparesis（ Diabetic Gastroparesis,DGP）. Methods： Mecobalamin and Mosapride DGP poor effect of combination therapy in patients 20 cases were randomly divided into observation group and control group. The control group continued mecobalamin and Mosapride combined treatment,observation group with α- lipoic acid combined Xiangsha Liujunzi original treatment regimen in the control group,based on a course of two weeks,both groups were given positive life way interfere with good control and blood glucose and insulin,were observed before and after treatment clinical efficiency,serum motilin（ motilin. MTL） levels and liver and kidney function changes,were observed before and after treatment. Results： Difference between the two groups clinical efficacy of two weeks after treatment was statistically significant（ P〈0.05）. The observation group was better than the control group. Serum levels were decreased in MTL groups（ P〈0.05）,and the decline value was more obvious in observation group（ P〈0.05） than the control group. Conclusion： The α- lipoic acid combined Xiangsha Liujunzi clinically effective treatment of refractory DGP is safe and well tolerated,may strongly anti- oxidative stress and α- lipoic acid,improve neurological deficits and Xiangsha Liujunzi suppression motilin secretion and accelerate gastrointestinal motility related.
