中药高位灌肠与汤剂联合西药治疗急性肾功能衰竭疗效观察及对血肌酐、尿素氮水平的影响

摘　　要：目的:探讨中药高位灌肠与汤剂联合西药治疗急性肾功能衰竭患者疗效观察及对血肌酐、尿素氮水平的影响,为临床诊断治疗提供一定的科学依据。方法:112例急性肾功能衰竭患者根据随机数字表法随机分为对照组和观察组,各56例。两组患者均给予抗感染、抗休克以及纠正酸碱平衡和水电解质等常规方法。对照组在常规治疗基础上应用碳酸氢盐血液透析治疗;观察组在对照组基础上结合中药高位灌肠与汤剂治疗。结果:观察组治疗后总有效率(94.64%)显著高于对照组(76.79%),且具有统计学差异(P<0.05);观察组治疗后血肌酐和血尿素氮水平显著低于治疗前及对照组治疗后,且具有统计学差异(P<0.05);观察组Scr降至正常时间、BUN降至正常时间及尿蛋白转阴时间均显著少于对照组,且具有统计学差异(P<0.05);观察组治疗后血清炎症因子(IL-1、IL-6、TNF-α)水平显著低于治疗前及对照组治疗后,且具有统计学差异(P<0.05);观察组住院时间显著短于对照组,且具有统计学差异(P<0.05)。结论:中药高位灌肠与汤剂联合西药治疗急性肾功能衰竭患者疗效显著,可明显降低血肌酐、尿素氮水平,明显缩短肾功能恢复时间及住院时间,具有重要临床研究意义。Objective： To investigate the effect of high decoction enema combined with Western medicine treatment of acute renal failure and its impact on efficacy in patients with serum creatinine,blood urea nitrogen levels,providing a scientific basis for clinical diagnosis and treatment. Methods： 112 patients with acute renal failure were randomly divided into a control group and observation group according to the random number table,both 56 cases. Two groups patients were treated with anti- infective,anti- shock and conventional methods to correct acid- base balance and water electrolytes. The control group used bicarbonate hemodialysis treatment on the basis of conventional therapy; observation group combined traditional Chinese medicine decoction high enema treatment. Results： The total effective rate（ 94. 64%） was significantly higher（ 76. 79%） after treatment with a significant difference（ P〈0. 05）; serum creatinine and blood urea nitrogen levels were observed after treatment group and were significantly lower than those before treatment and in the control group with a significant difference（ P〈0. 05）; observation group Scr,BUN to normal time and urinary protein clearance time were significantly less than those of the control group with a significant difference（ P〈0. 05）; observation group after treatment serum inflammatory cytokines（ IL- 1,IL- 6,TNF- α） were significantly lower than those pre- treatment and post- treatment in control group with a significant difference（ P〈0. 05）; observation group hospitalized time was significantly shorter than that in the control group with a significant difference（ P〈0. 05）. Conclusion： TCM decoction combined with western high enema treatment of acute renal failure in patients with significant effect can significantly reduce serum creatinine and blood urea nitrogen levels and significantly reduce renal function recovery time and hospital stay. It has important clinical significance.
