摘 要:目的探讨电针(EA)内关穴对大鼠体外循环(CPB)后心肌细胞超微结构和线粒体通透性转运孔(MPTP)的影响。方法成年雄性SD大鼠75只,随机分为五组(n=15):假手术组(S组)、CPB组、CPB+EA组、CPB+苍术苷组(CPB+Atr组)和CPB+EA+苍术苷组(CPB+EA+Atr组)。采用尾动脉插管灌注,右颈静脉插管引流建立CPB模型。S组仅行麻醉和动静脉插管;CPB组行CPB 2 h;CPB+EA组行CPB 2 h且在CPB期间使用电针刺激双侧内关穴;CPB+Atr组在CPB开始时静脉注射苍术苷5 mg/kg,随后处理方法同CPB组;CPB+EA+Atr组在CPB开始时静脉注射苍术苷5 mg/kg,随后处理方法同CPB+EA组。麻醉复苏后2 h,取右股动脉血测定心肌肌钙蛋白I(c Tn I)浓度和肌酸激酶同工酶(CK-MB)活性;取左心室心肌组织,电镜观察心肌细胞超微结构,行线粒体损伤评分,并用差速离心法分离线粒体,测定线粒体内Ca2+浓度和MPTP活性。结果与S组比较,其余各组血浆c Tn I浓度和CK-MB活性升高,心肌细胞超微结构损伤较重,线粒体损伤评分增加,线粒体内Ca2+浓度和MPTP活性升高(P<0.05);与CPB组比较,CPB+EA组和CPB+EA+Atr组c Tn I浓度和CK-MB活性降低,心肌细胞超微结构损伤较轻,线粒体损伤评分下降,线粒体内Ca2+浓度和MPTP活性降低(P<0.05)。苍术苷可抑制电针内关穴对心肌的保护作用(P<0.05)。结论电针内关穴可减轻大鼠体外循环后心肌细胞超微结构损伤,其机制可能与抑制线粒体内钙超载和减少MPTP的开放有关。Objective To investigate the effect of electroacupuncture at Neiguan point (EA) on myocardial ultrastructure and mitochondrial permeability transition pore (MPTP) in rats undergoing cardiopulmonary bypass (CPB). Methods Seventy-five adult male Sprague-Dawley rats were randomly divided into 5 groups (n=15 each):sham operation group (group S), group CPB, group CPB+EA, CPB+infuse atractyloside group (group CPB+Atr), and CPB+EA+infuse atractyloside group (group CPB+EA+Atr). Tail arteries and right jugular vein were cannulated for CPB. Rats in group S were anaesthetized and cannulated; rats in group CPB were subjected to CPB for 2 h; rats in group CPB+EA were subjected to CPB for 2 h and electro-acupunctured at Neiguan point during CPB;rats in group CPB+Atr and group CPB+EA+Atr were infused atractyloside 5 mg/kg at the beginning of CPB. Blood samples were taken from right femoral artery at 2 h after anesthesia resuscitation for determination of plasma cardiac troponin I (cT-nI) concentration and creatine kinase-MB (CK-MB) activity, and those animals were sacrificed to obtain the myocardi-al specimens from left ventricle. Electron microscope was used to observe the changes of mitochondrial ultrastructure, and the severity of mitochondria injury was scored. Mitochondria fractions were isolated by differential centrifugation to test the concentration of Ca2+and MPTP opening degree. Results Compared with group S, the plasma concentra-tions of cTnI and activity of CK-MB were significantly higher, the damage of myocardial ultrastructure was aggravat-ed, the score of mitochondrial injury, the concentration of Ca2+in mitochondrial and MPTP opening degree were signif-icantly increased in other four groups (P〈0.05). Compared with group CPB, the plasma concentrations of cTnI and ac-tivity of CK-MB were significantly lower, the damage of myocardial ultrastructure was attenuated, the score of mito-chondrial injury, the concentration of Ca2+in mitochondrial and
