针药结合治疗轻中度血管性痴呆患者83例

摘　　要：目的评价三焦针法结合黄地散方药治疗轻中度血管性痴呆患者的有效性,探讨优化治疗方案。方法将270例轻中度血管性痴呆患者,随机分为综合治疗组、针刺组和西药组,各组90例。综合治疗组采用三焦针法与黄地散方药联合治疗;针刺组采用三焦针法治疗;西药组采用安理申治疗,共治疗12周,采用ADAS-cog、ADL进行评价,并比较3组临床疗效。结果1治疗后3组患者ADL评分均较治疗前有显著改善,差异均有统计学意义(P<0.05);3组行为能力组间比较差异无统计学意义(P>0.05);2治疗后3组患者ADAS-cog评分均较治疗前有显著改善,差异均有统计学意义(P<0.05);3组治疗12周后,ADAS-cog评分、认知功能组间比较存在差异性(P<0.05),其中综合组优于针刺组和西药组(P<0.05),针刺组与西药组不存在统计学意义(P>0.05)。结论三焦针法配合黄地散方药可改善血管性痴呆患者远期的认知功能且提升血管性痴呆患者日常生活能力,值得临床推广应用。ABSTRACT：OBJECTIVE To evaluate the efficiency of integrated Sanjiao acupuncture therapy and Huangdi Powder in trea-ting mild-moderate vascular dementia,and investigate optimum therapy.METHODS 270 cases of mild-moderate vascular de-mentia were randomly assigned to comprehensive therapy group,acupuncture therapy group and western medicine therapy group on average with 90 cases in each.The treatment of integrated Sanjiao acupuncture and Huangdi Powder,Sanjiao acu-puncture and donepezil were respectively adopted in comprehensive therapy group,acupuncture group and western medicine group for 12 weeks.The paper evaluated these three groups by busing ADAS-cog and ADL,and compared the clinical therapu-tic effects of groups.RESULTS After treatment,the ADL scores of three groups enhanced obviously which showed the sta-tistical meaning（P 〈0.05）,as well as the ADAS-cog scores of three groups.The comparison of behavior ability in three groups had no statistical meaning（P 〉0.05）.There was statistical meaning for the comparison of ADAS-cog scores and cogni-tive ability in three groups 12 weeks after treatment（P 〈0.05）,and comprehensive group was better than western medicine group（P 〈0.05）.There was no statistical meaning for the comparison between acupunture group and western medicine group （P 〉0.05）.CONCLUSION The integration of Sanjiao acupuncture and Huangdi Powder could improve the long-term cogni-tive ability and daily life ability of patients with vascular demential,thus is worth a wide application in clinic.
