益气化瘀方预防子宫内膜异位症术后复发的临床研究

摘　　要：目的观察益气化瘀方预防子宫内膜异位症术后复发的疗效。方法选取2013年1月至2014年9月在湖北省中医院妇产科、江汉大学医院妇产科门诊就诊的子宫内膜异位症术后患者65名,随机分成中药组（32例）和西药组（33例）。中药组采用益气化瘀方,西药组采用孕三烯酮,3月为1个疗程,用1个疗程。分别于疗程结束和停药6月时观察患者用药的副反应、月经情况和停药后的复发率、痛经复现情况、妊娠情况、癌抗原CA125等变化。结果 1复发率、CA125值变化：2组无显著性差异（P〉0.05）。2术后痛经复现情况、术后妊娠率、药物副反应：2组具有显著性差异（P〈0.05）,中药组优于西药组。3用药期间的月经情况：2组具有显著性差异（P〈0.05-0.01）,中药组优于对照组。结论在复发率、降低CA125值方面益气化瘀方与孕三烯酮疗效相当。在提高妊娠率、降低痛经复现率和痛经评分方面益气化瘀方疗效明显优于孕三烯酮。在用药后益气化瘀方具有副反应发生率低,不造成闭经,调经助孕等优势。OBJECTIVE To explore the clinical effect of Yiqi Huayu Fang in preventing postoperative recurrence of endome-triosis.METHODS Sixty-five cases who visited gynecology and obstetrics department of both Hubei University of Chinese Medicine and Jianghan University after endometriosis operation from January 2013 to September 2014 were selected and ran-domly divided into Chinese materia medica group（32 cases）and western medication group（33 cases）.The former was treated with Yiqi Huayu Fang,while the latter was treated with Gestrinone capsules.Both groups were treated for one course,with 3 months as a course.Side effects and menstruation were observed after one course of treatment,and recurrence rate after medi-cation withdrawal,dysmenorrhea,gestation and level of CA125 were observed 6 months after medication withdrawal.RE-SULTS There was no significant difference between two groups in recurrence rate and changes in level of CA125 （P 〉0.05）. While significant differences could be found in two groups when it comes to dysmenorrhea,postoperative gestation rate and side effects related to medication（P 〈0.05）.And Chinese material medica group were much better than western medication group. When it comes to menstruation on medication,there was significant difference between two groups （P 〈0.05-0.01）.Com-pared to western medication group,Chinese material medica did not affect menstruation of cases in Chinese materia medica group.CONCLUSION Yiqi Huayu Fang has similar effects in decreasing recurrence rate and level of CA125 to Gestrinone capsules.And the former is much better in increasing chances of gestation,decreasing rate of dysmenorrhea and dysmenorrhea score than the latter.
