摘  要:目的:探讨HDCT高清(HD)和非高清(NHD)扫描模式显示不同内径冠状动脉支架的区别。方法:不同直径的乐普316L和L605支架各5枚,分别注入稀释后CT值为300HU(管电压120k V时测量)的碘对比剂。丙烯腈-丁二烯-苯乙烯树脂制成空心管状结构模拟冠状动脉,将支架分别置入每支模拟冠状动脉内,封闭开口后固定于一圆柱形塑料瓶外,并将瓶内注入同样CT值的对比剂溶液模拟心腔,再将模型静置于注满水的塑料盒内上机扫描。对支架体模分别进行HD模式和NHD模式扫描。采用前瞻性心电门控扫描,心电模拟规则心跳60次/min,仅采集75%R-R间期时相,采用HD Detail(HD组)和Detail(NHD组)模式后重建。测量所有支架内径、分别计算支架的平均内径测量值及支架腔内显示率,并进行统计学分析。结果:HD组支架内腔平均显示率为(49.2±8.9)%,NHD组支架内腔平均显示率为(45.9±10.0)%,两组差异有统计学意义(t=-5.250;P<0.05),HD组支架管腔显示率较NHD组高约7.2%。316L组支架平均显示率(50.3±7.3)%,L605组支架平均显示率(44.8±10.7)%,两组差异无统计学意义(t=1.350;P>0.05)。5枚316L支架中,两种扫描模式在直径2.5mm和4mm支架管腔显示率上差异无统计学意义(P=0.785和P=0.468)。结论:HDCT HD扫描模式对内径≥2.75mm且<4.00mm的冠状动脉支架的显示效果优于NHD扫描模式,对于显示内径<2.75mm和≥4.00mm的冠状动脉支架,两者无显著差异。Purpose:To explore the differences of image quality of coronary stent with various diameters between that acquired with high-definition(HD) and non-high-definition(NHD) scan mode by HDCT.Methods:Ten coronary artery stents of different material and diameter were studied.Five stents were made of stainless surgical steel(316L),five were made of cobalt-chromium alloy(L605).All stents were implanted in a coronary vessel phantom made of a plastic tube filled with contrast material.Images were acquired on a HDCT scanner,the Discovery CT750 HD,with two scan mode of high-definition and standard-definition.Prospective ECG-gating scanning mode was used,the simulate heart rate was 60 bpm,and 75%R-R interval images were acquired with HD Detail(HD) and Detail(NHD) mode reconstruction.Stent diameters were measured.The mean inner diameter and visibility of different type stents were calculated and compared between two groups.Results:There was a statistical difference of the measured inner stent diameter between HD group(49.2±8.9)%and NHD group(45.9± 10.0)%(P〈0.05).The visibility of HD group was 7.2%higher than that of NHD group.For stents diameter of 2.50 mm and 4.00 mm of 316 L,there was no statistical difference in visibility between them.Conclusion:HD scan mode of HDCT is better than NHD to visualize stents from 2.75 mm to 4.00 mm.For stents with diameter 〈2.75 mm or ≥4.00 mm,there is no significant difference between these two modes.
