多重耐药菌株医院感染监测及耐药情况分析

摘　　要：目的分析医院多重耐药菌(MDRO)株的感染情况及耐药情况,为医院制定MDRO的防控措施提供理论依据。方法采集2010年1月—2013年12月在该院住院部接受治疗的患者的血液、痰液、尿液以及分泌物作为标本,通过分离及药敏检查得到感染分布及耐药性。结果 MDRO菌株共检出15种,主要由大肠埃希菌(19.65%)、肺炎克雷伯菌(13.83%)、铜绿假单胞菌(12.92%)、鲍曼不动杆菌(12.37%)、溶血葡萄球菌(12.19%)以及表皮葡萄球菌(8.74)大比例构成。在送检的23 955份标本中共检出菌株5 184株,包括1 099株MDRO,占21.20%。在革兰阴性MDRO中对氨苄西林、头孢类的耐药性高,对阿米卡星较为敏感,尤其是大肠埃希菌、肺炎克雷伯菌。革兰阳性中主要对青霉素G、氨苄西林、苯唑西林等抗生素耐药性较高,对头孢类抗生素也表现出较强的耐药性。结论MDRO医院感染现象比较严重,并且MDRO的耐药性普遍较高,临床治疗以及护理应该做出相应的防范措施。Objective To explore multi drug resistant bacteria in hospital infection （MDRO} and resistant strains, provide theoretical basis for the prevention and control of hospital MDRO. Methods This study selected January 2010 to December 2013 in author＇s hospital treated patients blood, sputum, urine and secretions as specimens. The distribution and drug resistance of strains were obtained through the isolation and drug sensitivity test. Results MDRO strains were detected in 15, mainly in the Escherichia coli （ 19.65% ）, Klebsiella pneumoniae （ 13.83% ）, Pseudomonas aeruginosa （ 12.92% ）, Bauman aeinetobacter （12.37%）, Staphylococcus haemolyticus （12.19%） and Staphylococcus epidermidis （8.74）. In the CPC samples 23955 samples showed 5184 strains, including 1099 strains of MDRO, accounting for 21.20%. They were high resistance to Ampieillin, Cephalosporin for gramnegative MDRO, was sensitive to Amikacin, especially Eseherichia coli, Klebsiella pneumoniae. Gram positive mainly to Penicillin G, Ampicillin, antibiotics such as Oxacillin resistance to Cephalosporins was higher, also showed strong resistance. Conclusion MDRO infection in hospital is quite serious, and the resistance of MDRO is generally higher, clinical treatment and nursing should make corresponding prevention measures.
