桂枝去桂加茯苓白术汤对脓毒血症性急性肾损伤的疗效研究

摘　　要：目的探讨桂枝去桂加茯苓白术汤方治疗脓毒血症性急性肾损伤(AKI)患者的临床疗效。方法选择脓毒血症性AKI患者35例,随机分为实验组和对照组,通过检测两组治疗前后的血肌酐(SCr)、血清中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、尿NGAL、尿液肾损伤因子-1(u KIM-1)和尿素氮(BUN)水平,观察尿量和血白细胞计数,比较传统临床治疗方法和加用桂枝去桂加茯苓白术汤治疗的临床效果。结果治疗后实验组5项检测指标呈下降趋势,第3天血NGAL、尿NGAL和尿KIM-1明显下降(P<0.01),第5天SCr、BUN明显下降(P<0.01);对照组未见明显下降,甚至还有升高;治疗后第7天实验组尿量明显增加,对照组增加不明显;两组患者血白细胞计数明显下降,其中实验组更显著。结论在传统治疗方法基础上,加用桂枝去桂加茯苓白术汤能在较短时间内明显改善脓毒血症性AKI患者的肾功能。[Objective] To explore the clinical effects of Guizhi Qu Gui plus Fnling Baizhn Soup on acute kidney injury induced by sepsis （SAKI） in admitted patients. [Methods] Thirty-five patients of SAKI were enrolled and randomly divided into treatment and control groups. The clinical effects of Guizhi Qu Gui plus Fuling Baizhu Soup and the traditional clinical treatment were estimated by detecting the levels of serum creatinine （SCr）, plasma neutrophil gelatinase-associated lipocalin （pNGAL）, urine NGAL （uNGAL）, urine kidney injury molecule 1 （uKIM-1）, blood urea nitrogen （BUN）, and observing the urine volume and blood WBC count before and after treatment. [Results] The levels of pNGAL, uNGAL, uKIM-1 and BUN were in falling trends in the treatment group, and then the levels of pNGAL, uNGAL, uKIM-1 decreased obviously on the third day after treatment （P 〈 0.01）, the levels of SCr and BUN significantly reduced on the fifth day （P 〈 0.01）, but those in the control group did not decrease, even rose. The urine volume in the treatment group increased significantly on the seventh day, but that in the control group did not; and blood WBC count significantly decreased in both groups, which was more obvious in the treatment group. [Conclusions] Adding Guizhi Qu Gui plus Fuling Baizhu Soup on the base of the traditional clinical treatment could significantly improve renal function in a relatively short time in the patients of SAKI.
