布鲁杆菌病患者肝功能损伤临床观察

摘　　要：目的 了解人间布鲁杆菌病（布病）患者肝功能损伤临床特点,为临床治疗提供理论依据.方法 以2011年1月至2012年10月乌兰察布市地方病防治中心确诊收治的136名布病住院患者作为观察对象,回顾性调查分析患者病历中肝功能指标,包括患者血清丙氨酸氨基转移酶（ALT）、天冬氨酸氨基转移酶（AST）、碱性磷酸酶（ALP）、γ-谷氨酰基转移酶（γ-GT）、总蛋白（TP）、白蛋白（ALB）、白蛋白/球蛋白（A/G）、总胆红素（TBIL）、结合胆红素（DBIL）含量.肝功能指标调查结果与对应正常参考值比较,采用单样本t检验;急、慢性布病患者肝功能指标比较,采用两独立样本t检验.结果 136名布病患者血清ALT、AST、ALP、γ-GT、TP、ALB、TBIL、DBIL含量（面,36.18、28.20、95.87、29.29 U/L,65.08、40.84 g/L,19.76、5.86 μmol/L）与参考值（（x）,20.50、18.50、85.00、24.50 U/L,69.00、43.50 g/L,11.80、3.40 μmol/L）比较,差异有统计学意义（t=9.46、9.19、4.55、2.22、-2.71、-4.99、5.58、11.32,P均＜0.05）;血清A/G（（x）,2.14）与参考值（（x）,2.00）比较,差异无统计学意义（t=0.94,P＞ 0.05）.急、慢性布病患者肝功能各指标对比,差异无统计学意义（P均＞0.05）.结论 布病患者肝功能普遍受损且受损程度轻微或中等;急、慢性布病患者肝功能各项指标损伤程度未见明显差异.Objective To study the characteristics of liver function damage in brucellosis and provide theoretical basis for clinical treatment.Methods One hundred and thirty-six hospitalized patients with brucellosis from Endemic Disease Prevention and Control Center of Wulanchabu City from January 2011 to October 2012 were selected randomly as the research subjects.Parameters of Alanine transaminase （ALT）,Aspartate transaminase （AST）,Alkaline phosphatase （ALP）,Gamma glutamyltransferase （γ-GT）,Total protein （TP）,Albumin （ALB）,Albumin/globulin （A/G）,total bilirubin （TBIL） and Direct bilirubin （DBIL） were determined.The means of liver function parameters with mid-value of all the reference ranges were compared by one sample t test;Parameters of liver function were analyzed in 70 acute and 66 chronic brucellosis by two independent-samples t test.Results The means of ALT,AST,ALP,γ-GT,TP,ALB,TBIL and DBIL （（x）：36.18,28.20,95.87,29.29 U/L,65.08,40.84 g/ L and 19.76,5.86 μmol/L） in the 136 samples of brucellosis serum were significantly different from the medians of their reference ranges （（x）：20.50,18.50,85.00,24.50 U/L,69.00,43.50 g/L and 11.80,3.40 μ mol/L,t =9.46,9.19,4.55,2.22,-2.71,-4.99,5.58,11.32,all P 〈 0.05）;A/G was not significantly different （（x）：2.14,2.00;t =0.94,P 〉 0.05）;Parameters of liver function were not significantly different between acute and chronic brucellosis （all P 〉 0.05）.Conclusions This study confirmed that the liver function of the 136 patients with brucellosis is impaired.The level of the liver function impairment is usually mild and moderate and no difference in acute and chronic brucellosis is found.
