摘 要:目的分析204例登革病毒感染者的病毒血清型别分布及抗体产生特征,为研究登革热流行动态特征及临床诊治策略提供实验依据。方法采集登革热患者急性期血清,应用实时荧光PCR检测登革病毒核酸及分型,ELISA法检测登革病毒特异性抗体Ig G和Ig M。结果共检测204例标本,其中179例登革病毒核酸阳性,DENV-1型、DENV-2型及DENV-3型分别占97.2%(174/179)、1.1%(2/174)及0.6%(1/174);DENV-1合并DENV-2型感染和DENV-1合并DENV-3型感染各1例。ELISA法结果显示Ig M、Ig G抗体阳性者分别为193例和47例,且Ig G抗体阳性者的Ig M抗体同为阳性,表明初次感染和二次感染分别为146例(75.6%,146/193)和47例(24.4%,47/193)。结论广州存在多种血清型流行及二次感染患者,提示今后广东省重症登革热的发生几率可能会有所增加,应引起重视。Objective To analyze the serotypes of dengue viruses and specific antibodies from patients,to provide evidence for investigating the epidemiological dynamic characteristics and the strategies of clinical diagnosis and treatment of dengue fever in Guangzhou.Methods Real-time fluorescence polymerase chain reaction(PCR) was employed to detect the dengue virus nucleic acid and genotypes of the specimens collected from acute patients.ELISA was employed to detect the specific Ig G and Ig M for dengue virus.Results 204 samples were involved in real-time fluorescence PCR detection and 179 individuals showed positive results,with Genotyping result as the DENV-1(97.2%,174 / 179),DENV-2(1.1%,2 / 174) and DENV-3(0.6%,1 / 174).Two co-infections were found;one was DENV-1 with DENV-2 co-infection;another was DENV-1 with DENV-3co-infection.The results of enzyme linked immunosorbent assay(ELISA) showed that the number of positive cases of Ig G and Ig M was 47 and 193,respectively,and all Ig G positive cases were positive for Ig M,indicating that the number of primary infection and re-infection was 146(75.6%,146 / 193) and 47(24.4%,47 / 193),respectively.Conclusion There were multiple serotypes and secondary infections of dengue fever in Guangzhou,which indicated the incidence of severe patients would increase,and we should pay more attention to it.
