右正中神经电刺激对脑损伤术后昏迷患者催醒的临床研究

摘　　要：目的探讨右正中神经电刺激对脑损伤术后昏迷患者的临床疗效。方法选取颅脑损伤术后昏迷超过7 d、格拉斯哥昏迷评分（GCS）3~8分的脑损伤患者52例,随机分成对照组和治疗组,每组26例。两组均予常规治疗,治疗组在常规治疗的基础上加行右正中神经电刺激治疗,比较两组患者治疗2个月后GCS评分、JFK昏迷恢复量表评分（CRS-R）、脑电图（EEG）、脑干听觉诱发电位（BAEP）的变化情况。结果治疗2个月后,治疗组GCS和CRS-R评分分别为（11.01±1.19）和（8.62±1.34）,较对照组有明显改善（P〈0.05）;治疗组的EEG以及BAEP改善效果显著优于对照组（P〈0.05）。结论右正中神经电刺激可改善脑神经电生理活动,促进脑损伤术后患者苏醒。Objective To investigate the clinical benefits of right median nerve electrical stimulation on coma patients with traumatic brain injury after operation.Methods 52 patients who in coma for more than 7 days,Glasgow coma score（GCS）from 3 to 8 points,were randomly divided into control group（26 cases） and treatment group（26 cases）.The patients in two groups were treated with the routine medicine,and the patients in treatment group were treated with the right median nerve electrical stimulation additionally.The changes were compared between two groups after two months of treatment with GCS,CRS-R score,EEG and BAEP.Results After two-month treatment,the GCS score and CRS-R score were（11.01±1.19） and（8.62 ±1.34）,respectively,which were higher than those of the control group（P〈 0.05）.Test results showed improvement both in EEG and in BAEP after treatment（P〈0.05）.Conclusion The therapy of median nerve electrical stimulation can improve the electroencephalography in comatose patients with traumatic brain injury after operation,and it can help those patients hastening awakening from coma.
