佛山市三水区中老年人群尿酸水平的相关危险因素分析

摘　　要：目的分析佛山市三水区中老年人群高尿酸的发病率,及其与体重指数、血酯、胰岛素抵抗指数、血糖的相关性。方法通过尿酸、血糖、空腹胰岛素及血脂等生化检查调查佛山市三水区35岁以上中老年人群3 326名,以55周岁中位点分为中年组和老年组,每组再根据血尿酸(SUA)水平分为高尿酸组和尿酸正常组,分别对照各组性别、体重指数(BMI)、血糖(FPG)、总胆固醇(TCHO)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、胰岛素抵抗指数(HOMA-IR)水平。结果中年组和老年组高尿酸发病率分别为21.17%、32.11%,老年组显著高于中年组,差异有统计学意义(P<0.05)。中年组中高尿酸组BMI、FPG、TG、HOMA-IR显著高于尿酸正常组(P<0.01),且与性别相关;老年组中高尿酸组BMI、FPG、TG、HOMA-IR显著高于尿酸正常组(P<0.01),且与性别无相关。老年组中高尿酸组HOMA-IR显著高于中年组中高尿酸组,差异有统计学意义(P<0.01)。结论高龄、男性、高体重指数、高甘油三酯的人群中高尿酸发病率增高,高尿酸血症与胰岛素抵抗呈正相关。Objective To investigate the uric acid level among the middle age and old people in Sanshui district of Foshan city and its correlation with the body mass index,triglyceride,blood glucose,insulin resistance index.Methods 3 326 people over 35 years old were surveyed for the levels of uric acid,blood glucose,fasting insulin and blood lipid.Based on 55 years old as a cut off line,these people were into a middle-aged group and elderly group;each group according to the blood level of uric acid（SUA） was divided into high uric acid group and normal uric acid group,and control group.Gender,body mass index（BMI）,blood glucose（FPG） level,total cholesterol（TCHO） level,low density cholesterol（LDL-C） level,triglyceride（TG） level,and insulin resistance index（HOMA-IR） were compared among these groups.Results High uric acid rates of middle-aged group and elderly group were 21.17%,and 32.11%,respectively,and the rate of elderly group was significantly higher than that of middle-aged group（P〈0.05）.In the middle-aged group,the body mass index（BMI）,blood glucose（FPG）,triglycerides（TG）,and insulin resistance index（HOMA-IR） of people with high uric acid level were significantly higher than those of normal uric acid group（P〈0.01）,and was associated with gender.In the elderly group,body mass index（BMI）,blood glucose（FPG）,triglycerides（TG）,and insulin resistance index（HOMA-IR） of people with high uric acid level were than normal group（P〈0.01）,and was not related with gender.People of elderly group with high uric acid level had a significantly higher insulin resistance index of than those people of the middle-aged group with high uric acid level（P〈0.01）.Conclusion Advanced age,male gender,high body mass index,high triglycerides population increased incidence of hyperuricemia,hyperuricemia and insulin resistance were positively correlated.
