稳定型冠心病合并高同型半胱氨酸血症患者冠脉病变特点分析

摘　　要：目的 探讨稳定型冠心病合并高同型半胱氨酸血症（Hcy）患者冠脉病变特点.方法 选择2013年1月至2014年1月我院经冠状动脉造影确诊且除外急性冠脉综合征的稳定型冠心病（SCAD）患者106例,其中男性67例、女性39例,依据血浆Hcy水平分为高Hcy组（58例）和正常Hcy组（48例）.两组性别、年龄、血压、血脂等临床基础情况未见统计学差异,具有可比性.冠脉造影图像采用直径法分析血管狭窄程度,以血管支数法和Gensis评分评价冠脉病变程度.结果 与正常Hcy组相比,高Hcy组冠脉三支血管病变比例增高（36.20％比8.33％）,Gensis评分增高（39.97±6.29比31.09±4.41,P＜0.05）,差异有统计学意义.Hcy水平与冠脉病变血管支数、Gensis评分呈正相关（P.＜0.05）.结论 高血浆Hcy可进一步影响稳定型冠心病患者冠脉病变的范围和严重程度.Objective To investigate characteristics of coronary artery disease in the patients with stable coronary heart disease and hyperhomocysteinemia （Hcy）. Methods A total of 106 patients with table coronary heart disease were enrolled finally and divided into 2 groups according to the level of blood Hey： the high Hcy group （n=58） versus the normal Hcy group （n=48）. The baseline of the clinical conditions such as gender, age, blood pressure and lipids was comparable in the two groups. According to coronary angiographic images, the levels of coronary stenosis were analyzed by diameter analysis and the severity of coronary artery diseases was evaluated by the method of vascular count and Gensis score. Results The high Hcy group showed significantly increase in the branches of coronary lesion and Gensis score. The ratio of three branches of coronary lesions and Gensis score were higher in high Hcy group than those of the normal Hcy group （36.20% vs 8.33%, 339.97± 6.29 vs 31.09± 4.41,P〈0.05）. Correlation analyses revealed the presence of a positive correlation between the level of Hcy, branches of coronary lesion and Gensis score（P〈0.05）. Conclusion Hcy increases the scope and severity of coro- nary artery disease in the patients with stable coronary heart disease.
