血浆同型半胱氨酸和胱抑素C水平在老年脑梗死患者中的临床意义

摘　　要：目的观察老年脑梗死患者血浆同型半胱氨酸（Hcy）和胱抑素 C（Cys-C）的水平与神经功能缺损程度的相关性及临床意义。方法检测82例老年脑梗死患者血浆 Hcy 和 Cys-C 水平，并与60例非脑血管病患者作为对照组进行比较。根据美国国立卫生研究院卒中量表（NIHSS）评分，将脑梗死组分为轻、中、重度3组，并将各组各项指标之间进行比较。结果脑梗死组血浆Hcy、Cys-C 水平明显高于对照组，且临床分组血浆 Hcy、Cys-C 水平两两比较差异有统计学意义（P＜0．05）；脑梗死患者血浆 Hcy、Cys-C 水平与 NIHSS 评分呈正相关（r ＝0．54，P ＜0．05；r ＝0．45，P＜0．05）。结论血浆 Hcy、Cys-C 水平与脑梗死的严重程度密切相关，提示血浆 Hcy、Cys-C 水平的动态监测对脑梗死的预测和疗效观察有临床意义。Objective To observe plasma levels of homocysteine（Hcy）and cystatin C（Cys-C） in elderly patients with cerebral infarction and to investigate their clinical value as well as associations with neurologic deficit.Methods We compared plasma Hcy and Cys-C concentrations in 82 cases of cerebral infarction with those in 60 controls.82 cases of cerebral infarction were divided into three groups according to their National Institutes of Health Stroke Scale（NIHSS）scores,among which groups Hcy and Cys-C were compared.Results Mean plasma levels of Hcy and Cys-C in stroke patients were significantly higher than those of controls（P 〈0.05）and had positive correlation with NIHSS scores（r =0.54,P 〈0.05;r =0.45,P 〈0.05）.Moreover,Hcy and Cys-C concentrations were found significant differences among those three stroke groups（P 〈0.05）.Conclusion Plasma levels of Hcy and Cys-C were related with the severity of cerebral infarction,therefore it is of great clinical value to measure their dynamic concentra-tions.
