肺结核合并糖尿病患者外周血辅助性T 细胞17和白细胞介素-17A 检测及临床意义

摘　　要：目的研究肺结核合并糖尿病（PPTDM）患者外周血辅助性 T 细胞17（TH17）亚群及血清白细胞介素（IL）-17A 含量并分析其临床意义。方法收集肺结核合并糖尿病患者（PPTDM）、肺结核患者（PTB）及糖尿病（DM）患者、健康志愿者（HV）各30例，其中 PPTDM患者又分为先患肺结核后患糖尿病（PTB －DM）和先患糖尿病后患肺结核（DM－PTB）患者两组，各15例。流式细胞术检测 Th17细胞亚群，酶联免疫吸附试验法（ELISA）检测血清 IL-17A 含量，并分析其相关性。结果PPTDM组 Th17细胞亚群和血清 IL-17A 含量均显著高于 PTB 组、DM组和 HV 组（P ＜0．05）。DM-PTB 组 Th17细胞亚群和血清 IL-17A 含量均显著高于 PTB-DM组（P ＜0．05）。PTB 组、DM组和 HV 组之间 Th17细胞亚群含量差异无统计学意义（P ＞0．05）。PTB 组血清 IL-17A 含量显著高于 DM组和 HV 组（P ＜0．01）。DM组和 HV 组之间血清 IL-17A 含量差异无统计学意义（P ＞0．05）。PPTDM患者 Th17细胞亚群与血清中 IL-17A 呈正相关（P ＜0．05）。结论Th17细胞亚群和IL-17A在 PPTDM疾病进展中可能具有重要的作用。Objective To study the contents of Th17 cell and serum IL-17A in patients with pulmonary tuberculosis and diabetes mellitus （PPTDM） and to analysis their clinical significance. Methods 30 cases of tuberculosis patients with diabetes mellitus （PPTDM）,tuberculosis patients （PTB）,diabetes mellitus（DM）patients and healthy volunteers（HV）were collected,respectively.The PPTDMwere divided into PTB-DM and DM-PTB groups,15 cases in each.FCM was used to detect the content of Th17 cell,and ELISA was used to detect the content of serum IL-17A.Pearson correlation analysis was used to analyze their correlation of them.Results Th17 cell and serum IL-17A levels of PPTDMpatients were significantly higher than those in PTB,DMand HV groups（P 〈0.05）,respectively. Th17 cell and serum IL-17A levels of DM-PTB patients were significantly higher than those in PTB-DM patients（P 〈0.05）,respectively.There were no significantly differents of Th17 cell contents among PTB, DMand HV groups（P 〉0.05）.Serum IL-17A levels of PTB patients were significantly higher than those in DMand HV groups（P 〈0.05）,respectively.There was no significantly different of serum IL-17A levels between DMand HV groups（P 〉0.05）.The correlation analysis showed that there was a positively correla-ted between Th17 cell contents and serum IL-17A levels on PPTDM patients（P 〈0.05）.Conclusion Th17 cell and IL-17A may play an important role in PPTDMdevelopment.
