摘 要:目的研究肺炎克雷伯菌医院感染临床分布及其耐药状况,为感染预防控制和用药提供依据。方法通过临床标本检测与药敏试验,对某医院2010年1月-2013年12月临床送检标本进行病原菌分离鉴定和药敏试验,并对检验结果作出分析。结果连续4年从该医院临床送检标本中共分离出肺炎克雷伯菌3 137株,送检标本主要分布于重症监护病房、神经外科和呼吸科;菌株主要分离自痰液、尿液和血液标本。在分离的3 137株肺炎克雷伯菌中,产β-内酰胺酶(ESBLs)菌株占34.4%。耐药率排名前五位的抗菌药物为氨苄西林、头孢唑啉、呋喃妥因、氨苄西林/舒巴坦和头孢曲松。结论该医院肺炎克雷伯菌感染患者主要分布于重症监护病房,以呼吸道感染占主导地位,应有针对性地加强防控。Objective To investigate distribution of Klebsiella pneumoniae in hospital infection and drug resistance, so as to provide the basis for prevention and control and clinical application. Methods The bacterium identification and drug sensi- tivity test were conducted for pathogen isolation,identification and results analysis during 2010 -2013. Results A total of 3137 strains Klebsiella pneumoniae were isolated. The specimens mainly came from sputum, urine and blood. Mainly distrib- uted in ICU,neurosurgery and respiratory department. The rate of Producing ESBLs strains was 34.4%. The top five per- centages of antibacterial drugs were ampicillin, cefazolin, nitrofurantoin, ampicillin/Sulbactam and ceftriaxone. Conclusion Pneumonia klebsiella infection were mainly distributed in ICU and respiratory tract infection was significantly dominant, so we should strengthen prevention and control targetly.
