聚维酮碘冲洗联合左氧氟沙星滴眼预防白内障患者术后感染的疗效研究

摘　　要：目的探讨聚维酮碘稀释液结膜囊冲洗联合左氧氟沙星滴眼预防白内障术后感染疗效,并观察和分析其预后,为临床预防和降低白内障术后感染提供依据。方法选择2012年7月-2015年3月接受白内障手术患者364例、509只眼,按照单双号随机分为对照组和观察组,对照组患者180例、244只眼,术前应用左氧氟沙星滴眼进行预防;观察组患者184例、265只眼,应用聚维酮碘稀释液结膜囊冲洗联合左氧氟沙星滴眼进行预防,评估两组患者冲洗前、后及术后第2天细菌培养结果及术后不良反应。结果观察组患者冲洗后及术后第2天细菌阳性率均为0.75%,均低于对照组5.33%和6.15%,差异有统计学意义(P<0.05);观察组患者术后总不良反应发生率为0.75%,低于对照组3.28%,差异有统计学意义(P<0.05)。结论聚维酮碘稀释液结膜囊冲洗联合左氧氟沙星滴眼预防白内障术后感染具有较好的临床疗效,能够减少细菌培养阳性率,降低患者不良反应发生率。OBJECTIVE To explore the effect of povidone iodine diluent conjunctival sac irrigation combined with levofloxacin eye drops on prevention of postoperative infections in cataract patients and observe the prognosis so as to provide guidance for clinical prevention of the postoperative infections in the cataract patients .METHODS A total of 364 cataract patients （509 eyes） who received surgical procedures from Jul 2012 to Mar 2015 were enrolled in the study and randomly divided into the control group and the observation group according to the odd or even number ,the control group with 180 patients （244 eyes） was treated with levofloxacin eye drops before the surgery ,while the observation group with 184 patients （265 eyes） was given the povidone iodine diluent conjunctival sac irrigation combined with levofloxacin eye drops .The results of bacterial culture and the incidence of postoperative adverse reactions were observed before and after the irrigation and on the second day after the surgery .RESULTS The positive rates of the bacterial culture of the observation group were 0 .75% both after the irrigation and on the second day after the surgery ,lower than 5 .33% and 6 .15% of the control group ,and the difference was statistically significant （P〈0 .05） .The total incidence of postoperative adverse reactions of the ob‐servation group was 0 .75% , lower than 3 .28% of the control group , and the difference was statistically significant difference （P〈0 .05） .CONCLUSION The povidone iodine diluent conjunctival sac irrigation combined with levofloxacin eye drops can achieve significant clinical effect on prevention of the postoperative infections in the cataract patients , decrease the positive rate of the bacterial culture , and reduce the incidence of the adverse reactions .
