芬太尼联合喷他佐辛在剖宫产后自控镇痛的临床研究

摘　　要：目的:观察芬太尼联合喷他佐辛在剖宫产后自控镇痛的效果及安全性。方法:选择在本院进行剖宫产的孕妇136例为研究对象,将孕妇随机分为观察组和对照组,每组68例。两组均采取腰硬联合阻滞后行剖宫产术,术后给予静脉自控镇痛。结果:术后两组总体VAS评分比较,差异无统计学意义(P=0.145);两组各时点VAS评分比较无统计学差异(P=0.107);镇痛药与时间不存在交互作用(P=0.33)。观察组术后总体Ramsay评分低于对照组,差异有统计学意义(P=0.042);两组各时点Ramsay评分比较有统计学差异(P=0.027);镇痛药与时间存在交互作用(P=0.031)。两组产妇镇痛泵按压次数无统计学差异(P=0.137)。观察组不良反应发生率(5.9%)低于对照组(8.8%),但差异无统计意义(P=0.511)。结论:芬太尼联合喷他佐辛应用于剖宫产术后镇痛效果满意,且不增加不良反应。Objective：To observe the effect and safety of fentanyl combined with pentazocine in post cesarean section analgesia. Methods：A total of 136 cases of pregnant women in our hospital undergoing cesarean section were selected as the object of our study, then divided into observation group and control group, 68 cases in each group. The two groups were taken spinal epidural analgesia then undergoing cesarean section, after cesarean section intravenous patient-controlled analgesia. Results：Comparison of two groups of postoperative VAS score, the difference was not statistically signiifcant （P=0.145）;there wasn’t signiifcant differences between the two groups VAS score at each time point （P=0.107）;there wasn’t interaction between the analgesic and time （P=0.33）. hTe Ramsay score of the observation group were lower than those of control group, the difference has statistical signiifcance （P=0.042）. hTere was signiifcant differences between the two groups Ramsay score at each time point （P=0.027）;hTere was interaction between the analgesic and time （P=0.031）. hTere wasn’t statistical difference between maternal analgesia pump pressing numbers of the two groups （P=0.137）. hTe adverse reaction incidence of observation group was lower than that of control group, but there was no significant difference （P=0.511）. Conclusion：It is effective using Fentanyl combined with pentazocine in Post Cesarean section analgesia, and don’t increase the adverse reaction.
