耳压配合电针治疗中风后抑郁症31例

摘　　要：目的观察耳压配合电针治疗中风后抑郁症的临床疗效。方法选择中风后抑郁症患者60例,随机分为治疗组和对照组。治疗组31例采用耳压配合通督调神法电针,耳穴取神门、心、肝、脾、脑;电针百会、神庭、神门穴为主,配合辨证取穴,如:肝气郁结证取太冲,心脾两虚证取公孙,电针用疏密波,疏波频率5 Hz,密波频率50 Hz,强度为3.7 m A,留针时间30分钟;对照组29例用通督调神法电针,配合辨证取穴。两组均进行针对中风的一般常规治疗;两组患者均在治疗前及治疗6周后各应用HAMD(17项)、中医症候量表、罗纳普郎克睡眠积分表进行评分,以评价疗效。结果治疗6周后,两组HAMD评分、中医证候量表评分、罗纳普郎克睡眠积分表评分差异具有显著性统计学意义(P<0.05)。以HAMD评分标准判定疗效时,治疗组疗效优于对照组。结论耳压配合电针治疗中风后抑郁证,能全面改善患者的抑郁症状,提高患者的睡眠质量,疗效优于电针治疗的对照组。Objective To investigate the clinical efficacy of post‐stroke depression after ear acupoint with electroacupuncture therapy. Methods 60 PSD patients were randomly divided into treatment group and control group. In the treatment group, ear acupoint with elec-troacupuncture with dredging governer vessels and regulating mind therapeutic method was applicable to the 31 patients. Shenmen heart liver spleen and brain are selected as ear acupoints. Baihui Shenting and Shenben are selected as the main electroacupuncture points, with modification according to syndrome differentiation. In the control group, 29 patients received basic treatment with basic acupuncture and points and electroacupuncture the same with the treatment group. All patients got basic treatment including anti‐hypertension, anti‐diabet-ics and so on. Patients were evaluated by HAMD, TCM Rating Scale and Rhone‐Poulenc sleep score before treatment, 6 weeks after treat-ment to investigate the clinical efficacy. Results There is significant difference between the two groups in HAMD, TCM Rating Scale, and Rhone‐Poulenc sleep score, which has statistical significance （P〈0.05） after treatment. There are more positive effects on PSD patients in the treatment group, according to the control group, either evaluated by HAMD,TCM Rating Scale or Rhone‐Poulenc sleep score, which has statistical significance （P〈0.05） ,which is similar with each other （P〉0.05） . Conclution Ear acupoit with electroacupuncture has positive effects on post‐stroke depression, and can greatly improve the depressive condition, which has significant difference statically compared with the control group.
