腰背痛腰背筋膜水肿治疗前后MRI对比研究

摘　　要：目的初步探讨腰背痛腰背筋膜水肿患者治疗前后背侧腰椎棘突后方肌筋膜MRI信号表现特点,为临床诊断及治疗后随访提供参考。方法选取因腰背痛就诊病人进行MRI检查,随访进行了中医理疗、针灸、推拿按摩等保守治疗的患者;对比患者治疗前和治疗后一个疗程后或3个月后患者的背侧腰椎棘突后方肌筋膜的异常信号区进行对比分析,以对比观察其临床疗效。结果治疗1个疗程后或治疗3个月后,患者的症状、体征、日常生活情况等绝大部分均有明显的改善,复查MRI与治疗前的MRI图像比较,患者背侧腰椎棘突后方肌筋膜的异常信号区范围大多数有不同程度的缩小。结论中医理疗、针灸、推拿按摩等保守治疗对腰背痛腰背筋膜水肿患者的症状、体征、日常生活情况有明显改善作用(临床疗效显著);MRI检查对腰背痛腰背筋膜水肿患者的诊断、治疗后评估有明显指导作用。Objective Preliminary study signal characteristic of MRI on lumbodorsal pain of lumbar dorsal rear fascia around lumbar spinous pre and post treatment, and to provide a reference for clinical diagnosis and follow-up after treatment. Methods To summarize and analyze signal characteristic of MRI on lumbodorsal pain of lumbar dorsal rear fascia around lumbar spinous pre and post conservative treatment of physiotherapy, acupuncture and Tuina,and to observe the clinical curative effect. Results The patient’s symptoms, signs and the situation of daily life were improved after treatment for one course or 3 months, and the abnormal MRI signal area size of lumbar dorsal rear fascia around lumbar spinous was decreased inordinately comparing with pre treatment. Conclusion The conservative treatment of the Chinese medicine physiotherapy, acupuncture and Tuina of lumbar dorsal fascia edema in patients with low back pain symptoms, signs and the situation of daily life has improved significantly（clinical efficacy was significant）. The MRI examination played a clear guiding and role in the diagnosis and treatment evaluation for the patients with low lumbodorsal myofascial edema about lumbodorsal pain.
