自制中药敷料治疗烧伤残余创面的临床研究

摘　　要：目的:观察自制中药敷料治疗大面积烧伤后残余创面的疗效及安全性。方法:选取60处残余创面,采用自身配对对照的试验设计方法,将创面分为治疗组(30处创面,用自制中药敷料外敷换药)和对照组(30处创面,用利福平生理氯化钠溶液外敷换药)。记录首次换药后7、14 d的创面愈合率;在首次换药前及换药后7、14 d行创面细菌培养和疼痛程度评分,观察不良反应。结果:首次用药后7、14 d治疗组的创面愈合率均高于对照组,差异有统计学意义(P<0.05);首次用药后7、14 d,治疗组换药时的疼痛评分均低于对照组,其中,第7天与对照组比较,差异有统计学意义(P<0.05),第14天与对照组比较,差异无统计学意义(P>0.05);治疗组和对照组的创面细菌培养结果比较,差异无统计学意义(P>0.05);试验期间60处创面均未见与药物相关的不良反应。结论:本课题组自制的中药敷料能提高大面积烧伤后残余创面的愈合率,减轻换药时的疼痛,并具有一定的抑菌作用。Objective：To study the effect of Traditional Chinese Medicine dressing on the treatment of residual burn wounds. Methods： The were 60 cases of residual burn wounds divided into treatment group （30 residual burn wounds treated with Tradi- tional Chinese Medicine dressing） and control groups （30 residual burn wounds treated with Rifampicin Sodium Chloride Physi- ological Solution）, by matched controls. The healing rates, the visual analogue scales and the results of the bacterial culture were observed in the 7th day and the lgth day. The dressing-using relative adverse reaction during the experiment was also moni- tored. Results：The healing rate of the 7th day and the lgth day in the treatment group were higher than that of the control group, with statistically significant difference （P〈0.05）. The visual analogue scales of the wound pain in the treatment group were better than the control group, with statistically significant difference （P〈0.05） in 7th day and there was no statistically significant difference （P〈0.05） in 14th day. There was no obvious difference between the tratment and the group in the results of the bac- terial culture. The vast majority ofthe patients did not show dressing-related adverse reaction during the experiment. Conclusion： Application of traditional Chinese medicine dressing can improve the healing rates and reduce the pain feeling. Also it has the ability to anti infection.
