超声引导Seldinger法腹膜透析术对急性重症肾功能衰竭的疗效观察

摘　　要：目的探讨超声引导Seldinger法腹膜透析术在急性重症肾功能衰竭患者中的应用。方法 2014年1月至2015年1月,在床边超声引导下,采用Seldinger法经皮穿刺置入腹膜透析管行腹膜透析治疗患者15例。结果本组15例患者中,除1例术后发生管道阻塞,1例术后出现导管漂移,余13例腹膜透析管均成功放置到预定位置,顺利进行腹膜透析,术后7 d内无导管移位、网膜包裹、渗漏、腹膜炎、腹腔出血等并发症发生。结论 Seldinger法腹膜透析术安全、可行,术后并发症发生率低,缩短了手术操作时间,为抢救急危重患者争取了时间,值得临床推广应用。Objective To investigate the value of peritoneal dialysis treatments by implanting peritoneal dialysis tube with Seldinger method guided by bedside ultrasound in severe acute renal failure. Methods From January 2014 to January2015,peritoneal dialysis treatment was performed in 6 patients by implanting peritoneal dialysis tube with Seldinger method guided by bedside ultrasound. Results There was 1 patient had a outflow obstruction and 1 patient＇s catheter tip migrated to the right upper quadrant. The remain 13 patients were placed successfully to the desired location after peritoneal dialysis tube. Seven days after the surgery,there was no complication such as catheter displacement,Omentum,leakage,peritonitis and intraperitoneal hemorrhage for 13 patients. Conclusion This technique can effectively reduce hospitalization costs,pain of patients and operating time,and thus contributes to improving the rescue rate of severe patients.
