PFNAⅡ和DHS内固定治疗老年股骨转子间骨折的效果比较

摘　　要：目的比较PFNAⅡ和DHS内固定治疗老年不稳定型股骨转子间骨折的效果。方法选取2010年1月-2012年3月本院收治的148例股骨转子间骨折患者作为研究对象,随机分为PFNAⅡ组和DHS组,各74例。PFNAⅡ组给予PFNAⅡ治疗,DHS组给予DHS内固定治疗。比较两组的手术中切口长度、失血量,手术时间,术后行走能力等。结果 PFNAⅡ组的切口长度显著短于DHS组,失血量显著少于DHS组,差异有统计学意义（P〈0.05）。PFNAⅡ组的手术时间、透视时间显著长于DHS组,差异有统计学意义（P〈0.05）。PFNAⅡ组的股骨颈平均缩短距离、拐杖辅助下行走的平均时间显著短于DHS组,差异有统计学意义（P〈0.05）。PFNAⅡ组的术后4个月恢复到术前行走能力发生率显著高于DHS组,差异有统计学意义（P〈0.05）。结论 PFNAⅡ内固定治疗老年不稳定型股骨转子间骨折效果显著,值得临床推广应用。Objective To compare the effect of proximal femoral nail antirotation Ⅱ （PFNA Ⅱ） and dynamic hip screws （DHS） internal fixation in the treatment of senile unstable intertrochanteric fracture. Methods 148 patients with intertrochanteric fracture from January 2010 to March 2012 in our hospital were selected and randomly divided into the PFNA Ⅱ group and the DHS group,74 cases in each group.The PFNA Ⅱ group was given PFNA Ⅱ,the DHS group was given DHS.The length of incision,volume of blood loss during surgery,surgical time,postoperative walking capability in two groups was compared. Results The length of incision in the PFNA Ⅱ group was shorter than that in the DHS group,the volume of blood loss during surgery in the PFNA Ⅱ group was less than that in the DHS group,with signifi-cant difference （P〈0.05）.The time of surgery and fluoroscopy in the PFNA Ⅱ group was longer than that in the DHS group,with significant difference （P〈0.05）.The average shortening distance and the average time of the walking in the PFNAⅡ group was shorter than that in the DHS group,with significant difference （P〈0.05）.The incidence rate of the a-bility to walk before surgery in the PFNA Ⅱ group was higher than that in the DHS group,with significant difference （P〈0.05）. Conclusion The application effect of PFNA Ⅱ internal fixation in the treatment of senile intertrochanteric fracture is significant,it is worthy of clinical promotion and application.
