摘  要:目的探讨连续性静脉血滤(CVVH)在脓毒血症相关心衰患者中的疗效与安全性。方法选取2010年3月—2013年3月该院重症医学科发生脓毒血症出现心衰的住院患者25例为研究组,所有患者均采用CVVH治疗模式,观察其血滤前后心功能及各项临床指标的变化,并与单纯应用药物治疗重症感染致心衰患者16例(对照组)作对比。比较两组患者治疗前后心脏功能的变化。结果研究组患者病死率低于对照组,研究组患者经持续静脉血滤治疗后心功能及感染症状等均明显改善(P<0.05),而对照组则变化不明显,甚至病情加重,差异有统计学意义。结论 CVVH能够显著改善脓毒症相关性心衰患者心功能,疗效确切,无明显不良反应。Objective To investigate the efficacy and safety of continuous venovenous hemofiltration (CVVH) therapy in the treat-ment of heart failure associated with sepsis. Methods Twenty-five hospitalized patients with sepsis complicated by heart failure in the ICU of our hospital from March 2010 to March 2013 were selected as the study group, and all of them were given CVVH treatment. The cardiac function and various clinical indexes of the study group before and after hemofiltration were observed. Six-teen patients with heart failure due to severe infection were selected as the control group and treated only by drugs. And the car-diac function before and after treatment was compared between the two groups. Results Compared with the control group, the pa-tients in the study group had lower mortality. The cardiac function and symptoms of infection of the patients in the study group af-ter CVVH treatment improved significantly(P〈0.05), while those of the patients in the control group did not change significantly, or even worse. The differences between the two groups were statistically significant. Conclusion CVVH can remarkably improve the heart function of patients with heart failure associated with sepsis, and the effect is exact without significant side effects.
