摘  要:目的探讨一氧化氮吸入疗法在治疗新生儿持续肺动脉高血压的临床疗效。方法以该院2012年8月—2014年8月收治的46例持续肺动脉高压新生儿使用NO进行治疗作为观察组,另取32例PPHN患儿不使用NO治疗作为对照组,监测计算46例观察组患儿吸入前、吸入后30 min、吸入后12-24 h、吸入后36-72 h及停用后12h的平均气道压(MAP)、动脉血氧分压(Pa O2)、动脉导管前后相应部位(右上肢及左下肢)的经皮测血氧饱和度(Tc Sa O2)及肺动脉压力(PAP)等相关指标,并对两组患儿的临床疗效进行比较。结果观察组患儿在进行NO吸入治疗后动脉血氧分压(Pa O2)、动脉导管前后相应部位(右上肢及左下肢)的经皮测血氧饱和度(Tc Sa O2)均显著升高,吸入后30 min与吸入前差异有统计学意义(P〈0.05),同时平均气道压(MAP)与及肺动脉压力(PAP)开始迅速下降,且随着吸入时间的延长,各指标持续获得改善,直至停用后12 h,仍可保持良好的治疗效果。观察组患儿中痊愈28例,好转11例,治疗总有效率为84.8%,对照组患儿中痊愈19例,好转7例,有效率为81.2%,组间比较差异无统计学意义(P〉0.05)。结论 NO吸入疗法在治疗PPHN中起效快,效果好,副作用少且安全性高,可作为临床治疗PPHN的首选方法予以推广。Objective To observe the clinical effect of continuous inhaled nitric oxide therapy in the treatment of neonatal pul-monary hypertension. Methods 46 cases of patients with neonatal persistent pulmonary hypertension were selected in our hospital from August,2012 to August, 2014,they were treated using NO as the observation group, another 32 cases of PPHN in children without the use of NO treatment were used as control group, 46 cases in observation group were monitoring calculation before, after inhalation inhalation after inhalation of 30min, 12-24 h after inhalation mean airway 36-72h and 12h after cessation of pressure (MAP), arterial partial pressure of oxygen (PaO2), arterial catheter before and after the corresponding parts (right upper and left low-er limb) of the transcutaneous oxygen saturation (TcSaO2) and pulmonary artery pressure (PAP) and other related indicators, and clinical efficacy of the two groups of patients were compared. Results The observation group with the NO inhalation in arterial partial pressure of oxygen (PaO2), arterial catheter before and after the corresponding position (right upper and left lower limb) of the transcutaneous oxygen saturation (TcSaO2) were significantly increased after inhalation of 30min, showed significant difference (P〈0.05), and the suction before and mean airway pressure (MAP) pressure and pulmonary artery (PAP) began to decline rapidly, and with the extension of suction time, each index continuously improved, until the discontinuation of 12h, keep the good thera-peutic effect can be. In the observation group with 28 cases were cured, 11 cases improved, the total effective rate was 84.8%, control group 19 cases were cured patients, 7 cases were improved and the efficiency is 81.2%, the difference was not significant between the groups (P〉0.05).
