左炔诺孕酮宫内缓释系统治疗子宫内膜增生症的临床及内膜超微结构研究

摘　　要：目的该研究旨在探讨左炔诺孕酮宫内缓释系统(LNG-IUS)治疗子宫内膜增生症对子宫内膜超微结构改变的意义。方法收集该院2012年7月—2014年7月收治的45例子宫内膜增生症患者,行LNG-IUS治疗。分别于治疗第3、6、12月行超声检测、诊断性刮宫术,观察患者月经量改变情况、子宫内膜厚度的变化及病理改变,并对病理标本进行电镜观察,了解子宫内膜超微结构变化情况。结果 LNG-IUS治疗可使月经量明显减少,差异有统计学意义(P<0.05),子宫内膜厚度降低,差异有统计学意义(P<0.05),治疗后电镜表现为:内膜上皮纤毛细胞间隙增宽,胞内可见核碎片,内质网扩张,线粒体肿胀,嵴消失。分泌细胞数量较治疗前明显减少,微绒不发达,细胞核碎片状改变。间质细胞变大,细胞器丰富,核大,常染色质丰富,呈蜕膜样细胞。结论 LNG-IUS治疗子宫内膜增生症是安全有效的。Objective This study aimed to investigate the levonorgestrel-releasing intrauterine system （LNG-IUS） Endometrial hy-perplasia of the endometrium ultrastructural changes in treatment. Methods The patients in our hospital, OK LNG-IUS treatment of 45 cases of endometrial hyperplasia. 3,6,12 months of treatment, respectively ultrasound testing, diagnostic curettage, menstrual flow was observed in patients with change, a change in endometrial thickness and pathology, and pathology specimens electron mi-croscopy to understand the endometrium Supermicro structural changes. The results： LNG-IUS treatment can significantly reduce （P 〈0.05） menstrual flow, endometrial thickness lower （P 〈0.05）, the performance of electron microscopy after treatment： ciliated epithelial cells lining the gap widened, intracellular visible nuclear debris, endoplasmic reticulum, mitochondria swelling, cristae disappear. Compared with the number of secretory cells before treatment significantly reduced micro velvet underdeveloped, frag-mented nuclei change. Mesenchymal cells become large, rich organelles, nuclear, rich euchromatin, was decidua-like cells. Con-clusion LNG-IUS treatment of endometrial hyperplasia is safe and effective.
