摘  要:目的探讨声脉冲辐射力成像在甲状腺结节中的诊断价值。方法选择2012年7月—2013年7月在该院拟行手术治疗的45例甲状腺结节患者,根据病理分析结果分为良性组65例和恶性组38枚,所有患者接受VTQ检查,并计算SWV均值。结果该研究中,ROI深度5~37 mm,平均(17.8±6.8)mm;VTQ检测结果显示,恶性组及各病理分型SWV值均显著高于良性组(P〈0.05),两组间周围组织SWV值差异无统计学意义(P〉0.05)。绘制VTQ检测ROC曲线,VTQ检测最佳临界值为3.25 m/s,相应敏感度为88%,特异度为72%。结论VTQ可以作为甲状腺结节良恶性的诊断依据之一,虽然目前技术上还存在一定局限,但是随着设备的发展,VTQ会在颈部淋巴结诊断中发挥重要的作用。Objective To investigate the value of acoustic radiation force imaging in diagnosis of thyroid nodules. Method Choice 45 patient from2012.7 to 2013.7 who take operative treatment, 65 cases of benign group and malignant group 38 cases. The entire patients take VTQ check and calculate SWV value. Result In the study group, ROI depth of 5 ~ 37mm, average (17.8 + 6.8) mm; VTQ test results showed that the malignant group and different pathological type SWV value were significantly higher than those in benign group (P〈0.05) between the two groups in the surrounding tissue, no significant difference between SWV values (P〉0.05). To draw the VTQ detection of ROC curve, the optimal VTQ detection critical value is 3.25m/s, the corresponding sensitivity was 88%, specificity was 72%. Conclusion VTQ can be used as one of the basis of diagnosis of malignant and benign thyroid nodules, although technically there are still certain limitations, but with the development of equipment, VTQ will play an important role in the diagnosis of cervical lymph node.
