WBC、cTnI和hs-CRP联合检测在各型手足口病诊疗中的应用价值

摘　　要：目的探讨三种常用检测项目对手足口病患儿诊治的临床意义。方法对临床107例手足口病患儿,按病情分为三型,对其白细胞计数(WBC)、肌钙蛋白I(c Tn I)和超敏C-反应蛋白水平(hs-CRP)结果进行分析统计,并与42健康儿童进行比较。结果手足口病普通型与对照组比hs-CRP出现差异(P<0.05),WBC和c Tn I无差异;重症型与对照组比hs-CRP有明显差异(P<0.01),WBC和c Tn I开始有差异(P<0.05),重症型与普通型比较,只有c Tn I有差异,而且是明显差异(P<0.01);危重型无论与对照组比,还是与普通型、重症型比,WBC、c Tn I和hs-CRP均有明显差异(P<0.01);各型病例的死亡率统计中,重症型病例已经出现死亡病例,死亡率为6.25%,危重型死亡率为15.38%。结论各型患儿三项指标与病程发展的程度不完全一致,因此三项指标联合检测将有助于临床医师对患儿进行分型和诊断,其中c Tn I是早期筛查出重症型患儿的关键检测项目。Objective To evaluate the clinical significance of three common test items in the diagnosis and treatment of hand, foot and mouth disease （HFMD）. Methods The 107 HFMD cases were divided into three types according to children＇s clinical condition, and their white blood cell count （WBC）, troponin I （cTnI） and hypersensitive C-reactive protein （hs-CRP） results were analyzed statistically, while 42 healthy children as the control group. Results Compared to the control group, the hs-CRP level of the common type of HFMD was different （P 〈0.05）, WBC and cTnI had no difference; while the hs-CRP level of the severe type of HFMD was different significantly （P 〈0.01） ,and WBC and cTnI had little difference. Compared to the common group, only the cTnI was different, but also significant differences （P〈0.01）; the critical type regardless of the control group, or the common type and severe type ratio, the levels of WBC, cTnI and hs-CRP were significantly different （P〈 0.01）. Of mortality rates of various types, the mortality rate of severe type was 6.25%; while the mortality rate of critical type was 15.38%. Conclusion The three indicators of the degree of each type and progression in children are not exactly the same, so combined detection of the three indicators will help clinicians to diagnose children with type, of which cTnI is a key detecting item of early screening for children of severe type.
