摘　　要：目的 :探讨临床药师对住院患者药学监护的模式。方法 :临床药师参与1例高血压合并急性心梗,慢性支气管炎及肾功能不全患者的药物治疗,对患者在肾功能不全情况下的高血压药物、β-受体阻滞剂的使用等方面提出了参考意见,并提供个体化药学服务。结果 :临床药师通过全程的药学服务,为临床提供合理性建议。结论 :临床药师参与用药实践可以提高药物使用的规范性、合理性。Objective： To explore the model for clinical pharmacists to provide pharmaceutical care for inpatients. Methods： Through participating in drug therapy of a patient with hypertension complicated with coronary artery disease and chronic bronchitis and renal insufficiency, clinical pharmacist could propose suggestions for the use of antihypertensives and 13-receptor blockers in the case of renal insufficiency and provide individualized pharmacy service for patient. Results： Clinical pharmacist could give reasonable recommendations for clinic by monitoring the whole course of treatment. Conclusion： The standardization and rationality of drug use can be improved by the practice of clinical pharmacist participating in clinical medication.
