摘  要:目的:分期观察针刀治疗颈型颈椎病后颈椎间盘、颈肌的形态学变化,同时观察对引起椎间盘退变的主要炎症因子——肿瘤坏死因子(TNF-α)水平的调节,为针刀治疗颈型颈椎病机理研究提供实验依据。方法:选用兔龄(6±0.5)个月的健康雄性新西兰家兔进行造模,评估后随机分组:对照组、针刀干预A组(干预后1周)、针刀干预B组(干预后4周)。予以相应处理。A组在干预后第7天处死取材,B组在干预后第28天取材。测定相应指标。结果:针刀干预后,针切部位可见以巨噬细胞为主的针切反应性炎症变化,新生毛细血管存在,最后针切断裂部位由趋向于正常骨胳肌及筋膜结构结缔组织修复填充。同时,针刀干预可以显著减缓颈椎间盘老化、退变的速度,消除髓核向纤维环裂隙突出的趋势,退变的颈椎间盘产生的主要致痛因子TNF-α水平在治疗后显著降低(P<0.01),不仅起效迅速,即时治疗效果良好(治疗后1周,P<0.01),而且持续性治疗效果同样良好(治疗后4周,P<0.01)。结论:针刀松解了颈部挛缩的筋膜、肌肉等,建立了新的代谢环境,以巨噬细胞为主的无菌反应性炎变促进了组织修复和重建。同时针刀干预对退变颈椎间盘中的TNF-α有良好的抑制作用,能显著降低颈椎间盘的退变速度,这可能是针刀防治颈椎病的重要机制之一。Objective : To observe the histological variations of the degenerated cervical intervertebral discs and muscles impacted by acupotomy, at the same time to observe this solution how to adjust the level of TNF- α which existed in the degenerated nucleus pulposus and played the leading role of inflammation factors, in order to provide morphological data for the fundamental mechanism research on acupotomy medical therapeutics. Methods : 30 healthy male New Zealand rabbits at ( 6 ± 0.5 ) months age were chosen for modeling and then we evaluated the success of random grouping: control group, aeupotomv-intervened group A ( 1 weeks after intervention ), acupotomy-intervened group B ( 4 weeks after intervention ). There of them were treated simultaneously. Rabbits in Group A were put to death in 7 days after intervention as well as group B put to death in 28 days..Corresponding indexes would be measured. Results : After the intervention, it was presented that a sort of reactive inflammatory change with blood vessels centered and macrophage aggregation, afterwards, a series of reparative processes occurred and the reparative results had a tendency of normal skeletal muscle and fascial structures. At the same time, acupotomy intervention would significantly reduce the speed of degeneration of the cervical intervertebral discs and eliminate tend of the degenerated nucleus pulposus slipping into the fiber ring fractures. TNF- α level decreased significantly after treatment (P〈0.01)which produced by degenerated cervical intervertebral discs. It not only worked quickly ( 7 days after treatment, P〈0.01 ), but also had sustained effect ( 28 days after treatment, P〈0.01 ). Conclusion : Aeupotomy cut the spasm muscles on the neck, established a new metabolic conditions. Inflammatory reaction mainly by macrophages became to promote tissue repairing and reconstruction.
