摘　　要：目的:观察Rttinger前外侧小切口入路配合独活寄生汤加减在人工关髋节置换中应用疗效。方法:采用前外侧小切口入路行人工髋髋关节置换63例,随机分成服用独活寄生汤组、非服用独活寄生汤组。服用组术后配合独活寄生汤加减。对术后的结果进行统计学分析。结果:Harris评分显示:术前Harris评分从35.5分提高至6个月93.2分,终末随访时的97.4分。VAS评分显示:两组疼痛较治疗前都有明显缓解,应用独活寄生汤组的术后疼痛减轻效果更具有优势(P=0.007,P<0.01)。卧床时间上,应用独活寄生汤组为(23.0±0.71)d少于非用药组(30.2±4.81)d(P=0.035,P<0.05)。住院时间上,应用独活寄生汤组为(14.4±01.67)d少于非用药组(14.4±1.67)d(P=0.012,P<0.05)。患肢体肿胀消退时间,用药组(9.80±0.45)d少于非用药组(12.0±1.22)d(P=0.02,P<0.05)。患者均能于半年左右步行来院复查,复查时拍摄双髋正位和患髋侧位片,均提示假体位置佳,无松动、下沉等表现,随访过程中,所有患者均能生活自理,行走自如,对自己的恢复情况满意。结论:Rttinger前外侧小切口入路配合中药独活寄生汤组在缓解术后疼痛、住院时间、卧床时间及患肢肿胀消退时间相对于非用药组更具有优势。Objective ： To observe the clinical effect of Rottinger anterior lateral small incision approach combined with Duhuojisheng Decoction in artificial hip replacement. Methods： Used anterior lateral small incision approach for 63 cases in artificial hip replacement, The patients were randomLy divided into two groups, taking Duhuojisheng decoction group and the non use of Duhuojisheng Decoction group. Duhuojisheng Decoction was given to the patients postoperatively. Results ： The Harris score, Harris score improved from preoperative 35.5 to 6 months 93.2 points,97.4 points at the time of the final follow-up. The pain of two groups had obvious remission than treatment before, the Duhuojisheng Decoction group has more advantages （ P=0.007, P〈0.01 ）. In the bed time., Duhuojisheng Decoction group was （ 23 ± 0.71 ） days less than non medication group （ 30.2 ± 4.81 ） days （ P=0.035, P〈0.05 ）. In hospitalization time, Duhuojisheng Decoction group was （ 14.4±1.67 ）days less than non medication group （ 14.4±1.67 ） days （P=0.012, P〈0.05 ） In suffering from limb swelling time, Duhuojisheng Decoction group was （ 9.80 ± 0.45 ） days less than non treated group （ 12 ±1.22 ） （ P=0.02, P〈0.05 ） .After a half of year, patients were able to walk to the hospital to review, according to anteroposterior and lateral radiographs of the hip check, indicated that the： prosthesis position was good, no loosening, subsidence, during the follow-up, all patients were able to self-care, walk freely and satisfied with his recovery. Conclusion ： Rottinger anterior lateral small incision combined with traditional Chinese medicine Duhuojisheng Decoction group has more advantages in relieving postoperative pain, hospitalization time, bed time, suffering from limb swelling time, compared with non treated group.
