摘 要:目的:验证成都市县级医院临床重点专科评估指标体系的实用性,查找成都市县级医院临床重点专科建设中存在的问题,并提出建设对策。方法运用成都市县级医院临床重点专科评估指标体系对成都市某县医院的16个临床专科进行评估,根据得分高低进行专科排名和频数分布,并从指标的得分率、难度系数、区分度、整体情况4个方面对评估结果进行综合分析。结果成都市某县医院16个临床专科得分为344.70~918.10分,平均598.69分。一级指标的得分率为32.29%~73.44%,平均55.16%;难度系数为0.2656~0.6771,平均0.4484;区分度为0.6638~0.9031,平均0.7576。结论评估结果可信度高,指标体系实用性强,针对重点专科建设中存在的管理不规范、专科带头人缺乏、人才队伍结构不合理、医疗技术水平落后、患者满意度不高、科研教学薄弱等问题,建议从资金投入、临床路径、人才培养、队伍建设、开拓创新、质量管理、合理用药、科研教学等方面加以改进提高。Objective To verify the practicability of the evaluation index system of clinical key specialty in Chengdu county hospitals, and to find out the existing problems in order to put forward construction countermeasures. Methods The evaluation index system of clinical key specialty in Chengdu county hospitals was used to evaluate the 16 clinical specialties, which were ranked and frequency distributed according to the scores, and made a comprehensive analysis of the results of the assessment in 4 aspects of the index score, degree of difficulty, discrimination and the overall situa-tion. Results In a Chengdu county hospital, 16 clinical specialties' scores were 344.70-918.10 points, with an average of 598.69 points. The one class scoring rate was 32.29%-73.44%, with an average rate of 55.16%, the difficulty factor was 0.2656-0.6771, with an average of 0.4484, and the discrimination was 0.6638-0.9031, with an average of 0.7576. Conclusion The evaluation result has high reliability and the index system is practical. Aiming at the existing problems of the construction of key specialty in management is not standardized, the lack of college leaders, personnel structure is irrational, the level of medical technology is backward, patient satisfaction is not high, scientific research and teach-ing is weak, there are suggestions to improve from capital investment, clinical path, personnel training, team building, innovation, quality management, rational drug use, scientific research and teaching etc.
