正常骶髂关节 MR DWI及 DCE 序列表现的相关性研究

摘　　要：目的：通过对健康志愿者骶髂关节（SIJ）旁骨髓区 DWI 及 DCE 序列相关参数的测量,探讨 ADC 值与 Fenh、Senh 值之间的相关关系,为 SIJ 相关病变的研究奠定基础。方法选取21例（男13例,女8例）健康志愿者行 SIJ MR DWI 及 DCE-MRI 扫描,获得 ADC 值平均值范围,绘制 TIC 曲线并分析曲线的类型,分析所得的 ADC 值与 Fenh、Senh 值及达峰时间 Tmax 值之间的相关关系。结果 SIJ 骨髓区在 b 值为600 s/mm2时 ADC 平均值左、右及骶、髂两侧骨髓区 ADC 值无明显差异（P〉0.05）。DCE 序列骨髓区 TIC 曲线有3种类型,8.3%（7/84）Ⅰ型,8.3%（7/84）Ⅱ型,83.3%（70/84）Ⅲ型；Fenh、Senh 值平均值均处于20%〈Fenh〈90%及10% min〈Sen〈40% min 范围内,左、右及骶、髂侧骨髓区所得到的 Fenh、Senh 及 Tmax 值的差异在统计学上均无明显意义（P〉0.05）,ADC 值与 Fenh 值之间有正相关。结论建立了正常 SIJ 骨髓区 ADC 值及 Fenh、Senh 平均值,明确了 ADC 值与Fenh 值之间呈正相关关系,为诊断早期 SIJ 病变提供了新的思路和方法。Objective To study the correlation between the ADC value and Fenh value,Senh value based on DWI and DCE-MRI, by measuring them in sacroiliac joint bone marrow of healthy volunteers.Methods 21 healthy volunteers performing the sacroiliac joint MR were involved.ADC value based on DWI,and TIC based on DCE-MRI were measured.The relationship of ADC values with Fenh,Senh values and peak to time,Tmax values were analyzed.Results The differences of the measured ADC values on both sides of the sacroiliac joint bone marrow region were not statistically significant （P〉0.05）.There were three types of TIC curve,8.3% （7/84）Ⅰ type,8.3% （7/84）Ⅱ type,83.3% （70/84）Ⅲtype;Fenh,Senh average values were within 20%〈Fenh〈90% and 10% min〈Senh〈40% min,the differences of the derived variables Fenh,Senh and Tmax values on left and right sides and the iliac,sacral bone marrow region of the sacroiliac joint were not statistically significant（P〉0.05）.There was a positive correlation between ADC value and Fenh value in bone marrow region of sacroiliac joint.Conclusion This study determined the average value of ADC and Fenh,Senh in normal sacroiliac joint bone marrow area,the positive correlation between ADC value and Fenh value is a new thought and may be a new method of diagnosising the early disease of sacroiliac joint.
