子宫缩减成形术合并促性腺激素释放激素激动剂治疗弥漫型子宫腺肌病的研究

摘　　要：目的探究子宫缩减成形术合并促性腺激素释放激素激动剂治疗弥漫型子宫腺肌病的效果。方法选取2012年5月到2014年5月在我院接受治疗的患有弥漫型子宫腺肌病的80例患者作为研究对象,两组各40例,治疗组采用子宫缩减成形术,对照组采用单纯子宫腺肌病病灶切除术,手术后均给予促性腺激素释放激素激动剂治疗。术后6个月后进行复查。结果治疗组临床有效率为97.5%,高于对照组82.5%,具有统计学意义(P<0.05);治疗组无复发,优于对照组的5例(12.5%)复发,具有统计学意义(P<0.05)。治疗组月经量正常者38例(95%),对照组月经量正常者34例(85%),治疗组优于对照组,具有统计学意义(P<0.05)。治疗组子宫体积降为(168.35±38.21)cm3,对照组子宫体积降为(181.68±37.89)cm3,经比较,治疗组小于对照组,具有统计学意义(P<0.05)。结论子宫缩减成形术合并促性腺激素释放激素激动剂治疗弥漫型子宫腺病疗效显著,值得在临床上推广使用。Objective To explore the effect of uterine reduction plasty combined with gonadotropin releasing hormone agonist in the treatment of diffuse adenomyosis. Methods 80 patients with diffuse adenomyosis were enrolled in this study including treatment group（ 40 cases） and control group（ 40 cases）. The treatment group was treated by uterine reduction plasty combined with gonadotropin releasing hormone agonist,and the control group was treated by simple adenomyosis foci resection combined with gonadotropin releasing hormone agonist. Their clinical results were reviewed 6 months after the operation. Results The effort of treatment group was better than control group. The clinical efficiency of the treatment group and the control group was 97. 5% vs. 82. 5% respectively（ P 〈0. 05）. The recurrence rate of the treatment group and the control group was 0 vs. 12. 5% respectively（ P 〈0. 05）. The treatment group had 38 cases of normal menstrual quantity,accounting for 95% of the number of the treatment group,and the control group had 34 cases of normal menstrual quantity,accounting for 85% of the total number（ P 〈0. 05）. The treatment group of uterine volume reduced to（ 168. 35 ± 38. 21） cm3,and the control group of uterine volume reduced to（ 181. 68 ± 37. 89） cm3（ P 〈0. 05）. Conclusion It shows significantly curative effect of the uterine reduction plasty combined with gonadotropin releasing hormone agonist treatment for the diffuse uterine adenosis patients,and is worth promoting in clinical use.
