孕期携带HBV对母婴结局的影响

摘　　要：目的研究孕妇携带乙型肝炎病毒(HBV)对母婴结局的影响。方法选取2011年1月至2013年12月在南方医科大学中山市博爱医院产检及分娩的乙肝表面抗原(HBsAg)阳性的孕产妇124例为研究组,选取同期产检及分娩的乙肝表面抗原(HBsAg)阴性的健康孕产妇124例为对照组,比较两组不良妊娠结局的发生率。结果 1研究组中孕产妇发生妊娠期高血压疾病、妊娠期糖尿病、妊娠期肝内胆汁淤积症、肝功能损害及产后出血的概率均明显高于对照组,差异有统计学意义(P均<0.05);2研究组中发生胎儿窘迫、新生儿小于胎龄儿、新生儿肺炎、新生儿高胆红素血症的概率均明显高于对照组,差异均有统计学意义(P均<0.05);3研究组中单阳组的母儿并发症与对照组比较差异无统计学意义(P均>0.05);4研究组中双阳组母儿并发症明显高于单阳组,差异均有统计学意义(P均<0.05)。结论孕期携带HBV可明显增加孕妇妊娠期及分娩期并发症的发生率,同时增加胎儿窘迫、新生儿肺炎及新生儿高胆红素血症的发生率,对母婴影响较大,临床上要引起重视。Objective To investigate the effect of hepatitis B virus （HBV） infection on the maternal and infant outcomes of pregnant women. Methods 124 pregnant women with HBV infection who treated in Zhongshan Boai Hospital affiliated to Southern Medical University from January 2011 to December 2013 recruited as observation group, and 124 healthy pregnant women in the same period were selected as control group. The incidence of adverse pregnancy outcomes were compared between the two groups. Results （1） The incidence of hypertensive disorder complicating pregnancy （HDCP）, gestational diabetes mellitus （GDM）, intrahepatic cholestasis of pregnancy （ICP）, liver function damage and postpartum hemorrhage （PPH） in the research group were significantly higher than that in control group （P〈0.05） ; （2） The rates of fetal distress, small for gestational age （SGA）, pneumonia of newborn and hyperbilirubinemia of newborn in the research group were significantly higher than that in control group （P〈0.05） ; （3） The mother and neonate complications in the one positive had no significant difference group compared with the control group; （4） The mother and neonate complications in the two positive group were signifi- cant higher than that in the one positive group. Conclusions Pregnancy with HBV infection can increase the inci- dences of complications during pregnancy and delivery. Furthermore, pregnancy with HBV infection can also in- crease the incidences of fetal distress, pneumonia of newborn and hyperbilirubinemia of newborn thus damage mater- nal-infant. We should pay attention to pregnancy with HBV infection.
