摘　　要：目的构建某区基于信息化医防结合的糖尿病防治模式的应用,并对其管理效果分析。方法采用文献评阅、定量分析和定性访谈等方法,描述某区糖尿病管理模式的演变过程,分析糖尿病管理模式转变前后相关指标的变化。结果基于信息化医防结合的糖尿病防治模式,改变以往管理效率低下、管理不连续等状况,患者管理数量和管理效率有明显的提高。结论以信息化为基础,实践临床预防结合在糖尿病管理中的应用,全面推进慢性病健康管理。Objective To evaluate the effectiveness of management model of diabetes mellitus based on the electronic health record and combination of clinic and prevention in MinHang district. Methods The methods as literature review, data collection and qualitative research were employed to discuss the change of management model of diabetes mellitus. , the evaluation indices were involved. Results The quality and quantity of diabetes mellitus management were remarkably increased. Conclusion The new management model of diabetes mellitus based on the electronic health record and combination of clinic and prevention was feasible and effective.
