丹红注射液对非体外循环冠脉搭桥围术期炎症反应的影响

摘　　要：目的:考察丹红注射液对非体外循环冠脉搭桥围术期炎症反应的影响。方法:将我院2012年9月-2014年1月收治的急性冠脉综合征患者76例采用随机数字表法分为治疗组和对照组各38例。治疗组术前静脉滴注丹红注射液40 ml+5%葡萄糖注射液210 ml,对照组术前静脉滴注5%葡萄糖注射液250 ml,分别测定两组患者麻醉前(T1)、手术后1 d(T2)、手术后4 d(T3)与手术后7 d(T4)血清超敏C反应蛋白(hs-CRP)的质量浓度,并比较两组患者手术后的恢复情况。结果:与T1比较,两组患者在T2、T3、T4时hs-CRP的质量浓度均明显增高,差异有统计学意义(P<0.05);治疗组患者在T2、T3、T4时hs-CRP的质量浓度均明显低于对照组,差异有统计学意义(P<0.05)。治疗组患者的呼吸机使用时间与术后肝功能损伤、肾功能损伤、肺功能损伤、中枢神经系统损伤及房颤发生几率与对照组比较均明显减少,差异有统计学意义(P<0.05)。结论:丹红注射液可用于临床治疗非体外循环冠脉搭桥术后引起的炎症反应并减少术后并发症。OBJECTIVE： To investigate the influence of Danhong injection on perioperative inflammatory response in off-pump coronary artery bypass surgery. METHODS： 76 patients with acute coronary syndrome in our hospital from Sept. 2012 to Jan. 2014 were randomly divided into treatment group （n=38） and control group （n=38）. Before treatment, treatment group was given Dan- hong injection 40 ml and 5% glucose injection 210 ml, ivgtt; control group was given 5% glucose injection 250 ml, ivgtt. The mass concentration of serum high-sensitivity C-reactive protein （CRP） in 2 groups before anesthesia （T1）, 1 d after surgery （T2）, 4 d after surgery （T3） and 7 d after surgery （T4） were respectively determined and the postoperative recovery of patients was compared. RE- SULTS： Compared with T4, the mass concentration of serum high-sensitivity CRP in T2, T3 and T4 in 2 groups were significantly in- creased, with significant difference （P〈0-05） ; the mass concentration of serum high-sensitivity CRP in T2, T3 and T4 in treatment group were significantly lower than control group （P〈0-05）. Compared with control group, the respirator time and the cases of liver dysfunction, kidney dysfimction, pulmonary dysfunction, central nervous system damage and atrial fibrillation after surgery in treatment group were significantly decreased, with significant difference （P〈0-05）. CONCLUSIONS： Dahong injection can be used for the treatment of inflammatory response induced by off-ptunp coronary artery bypass surgery and reduce the postoperative complications.
