摘  要:目的探究胎心监护在围产期的临床应用价值。方法选取2012年4月至2014年3月期间来我院的孕妇582例,随机分成两组,观察组采用产前无应激胎心监护(NST),产时电子胎心监护,对照组采用传统胎心听诊诊断,对比分析这两种监测方法的诊断正确率。结果两组孕妇的基本资料无明显差异(P>0.05)。观察组的剖宫产率和胎儿宫内窘迫诊断率均大于对照组,具有明显差异(P<0.05),观察组新生儿窒息率显著小于对照组,有统计学差异(P<0.05)。观察组围产儿死亡率为0,对照组有2(0.69%)例死亡,两组死亡率相比无显著差异(P=0.157)。291例观察组孕妇NST诊断结果显示,有反应型225例,占77.32%;可疑型45例,占15.46%;无反应型21,占7.22%。无反应型孕妇的胎儿窘迫、新生儿窒息、脐带异常的发生率均明显高于有反应型孕妇的发生率,差异具有统计学意义(P<0.05)。结论产前无应激胎心监护和产时电子胎心监护能够准确的诊断孕妇宫内异常,降低新生儿窒息,提高新生儿质量。与传统听诊胎心诊断相比,其准确性更高,适合临床使用。Objective: To investigate the clinical value of fetal heart rate monitoring in the perinatal period. Methods: 582 cases of pregnant women came to our hospital from April 2012 to March 2014 were randomly divided into two groups, the observation group prenatal were observed by no stress test of fetal heart rate monitoring (NST) , electronic fetal heart rate monitoring during the Intrapartum, control group were observed by traditional fetal heart auscultation diagnosis, compare and analysis the diagnostic accuracy of the two monitoring methods. Results: There was no significant difference (P〉0.05) of the basic information between the two pregnant groups. The cesarean section rate and the diagnosis of fetal distress rates in the observation group were obvious higher than the rates in control group, the results have significant differences (P〈0.05) , neonatal asphyxia rates in observation group was significantly lower than it in the control group, with a statistically significant difference (P〈0.05) . Perinatal mortality rates was 0 in observation group, while 2 (0.69%) children died in control group, the mortality rates showed no significant difference between the two groups (P = 0.157) . 291 cases of pregnant women who were by NST in the observation group, the results showed that the reaction type 225 cases, accounting for 77.32%: suspicious type 45 cases, accounting for 15.46%; non-reactive 21 cases, accounting for 7.22%. fetal distress, neonatal asphyxia, umbilical cord abnormalities with non-reactive maternal occurred significantly higher than the incidence of pregnant women with reaction type, the difference was statistically significant (P〈0.05) . Conclusion: non tress test of fetal heart rate monitoring during prenatal and electronic fetal heart rate monitoring in intrapartum can accurately diagnose intrauterine abnormalities, reduce neonatal asphyxia, improve the quality of neonatal.
