超声诊断各类孤立性侧脑室扩张胎儿0~6岁智能及体格发育情况分析

摘　　要：目的分析不同程度及转归的孤立性侧脑室扩张（IV）与生后0~6岁智能、体格发育的相关性。方法选择2007.1.1~2013.1.1于济宁医学院附属医院行超声发现IV的患儿186例为研究组。选择同期于本院受检的0~6岁正常婴幼儿200例为对照组。两组进行丹佛发育筛查（DDST）和体格发育测量,将不同程度及转归的研究组分别与对照组比较。结果轻度扩张的进展型、中度扩张的进展型和稳定型、重度扩张的3种类型与对照组差异有统计学意义。DDST异常及可疑者与年龄呈正相关。两组体格发育差异无统计学意义。结论轻度扩张的进展型、中度扩张的进展型和稳定型、重度扩张的3种类型可导致智能发育障碍,随着年龄增加而增多,对于早期未发现异常的高危儿应重视远期随访。IV对体格发育无影响。Objective. To study the relationship between the different degree and prognosis of isolated ventriculomegaly （IV） and the intelligential and physical development aged from 0 to 6 years old. Method- 186 IV cases were selected as the study group, who came between 2007.1.1 and 2013.1.1, in Affiliated Hospital of Jining Medical College. 200 nomal cases aged 0~6 years old came from the same hospital and period were selected as the control group. Two groups were given the Denver development screening test （DDST） and measurement of physical development. Each of the study group was compared to the control group. Results： Some parts of the study group had significant difference with the control group, which were the progressive of mild dilatation, the progressive and stable of moderate expansion, the 3 types of severe dilatation. DDST abnormal and suspicious cases were positively correlated with the age. The two groups had no significant difference in the physical development. Conclusion, Some parts of the study group can lead to mental retardation, which includ the progressive of mild dilatation, the progressive and stable of moderate expansion, the 3 types of severe dilatation. Older Infants ends up with more significant performance with younger ones. Infants in high risk who are not found abnormal early should be paid attention to and long-term follow-ups are necessary. IV had no effect on physical growth.
