摘 要:目的利用生物电阻抗频谱分析法(BIA)了解维持性血液透析(MHD)患者的体液分布特点,同时了解其对血压的影响。方法选择MHD患者64例,透析时间均≥3个月,根据透析间期血压控制情况,分为正常血压组(41例)与高血压组(23例)。应用BIA测定透析前细胞内液(ICW)、细胞外液(ECW),分别计算占体重的百分比(ECW%、ICW%)。另外选取64例性别、年龄、体重匹配的正常个体作为正常对照组。结果 MHD患者透析前ECW%明显高于同性别正常对照者(P<0.01),MHD患者高血压组透前ECW%明显高于同性别正常血压者(P<0.05)。结论透析患者的体液分布存在明显异常,主要表现为透析前ECW%增加。MHD伴难治性高血压者ECW%明显高于血压控制理想者。Objective To evaluate the fluid distribution in patients on maintenance hemodialysis(MHD)by the whole-body bioimpedanc spectroscopy and investigate its effect on blood pressure in MHD patients. Methods A total of 64 patients with stable MHD for more than 3 months were enrolled in this study. Based on inter-dialysis blood pressure control,they were divided into ideal blood pressure group(41 cases)and refractory hypertension group(23 cases). Preand post-dialysis intracellular water(ICW)and extracellular water(ECW)were measured by whole-body multifrequency bioimpedance spectroscopy,and standardized by body weight to produce ICW% and ECW% . 64 normal individuals of matched sex,age and body weight were used as controls. Results The pre-HD ECW% was significantly higher than those of normal controls(P 〈 0. 01). Patients with MHD had higher pre- and post-dialysis ECW% values than those without hypertension. Conclusion MHD patients have higher pre-dialysis ECW% . MHD patients with hypertension are significantly higher than the ideal blood pressure control group.
