醋酸联合靛胭脂色素内镜在上消化道病变诊断中的应用价值

摘　　要：目的探讨醋酸联合靛胭脂色素内镜在上消化道病变中的应用价值。方法 216例患者行常规内镜检查,对60例患者胃黏膜可疑病灶的病变中央及周边进行多处活检;然后进行醋酸-靛胭脂染色,比较常规内镜和色素内镜下病灶边界清晰程度,并于染色病变周边活检。另外176例患者随机分为2组,78例行常规胃镜检查,于病变边缘活检行病理检查;78例行胃镜下醋酸-靛胭脂染色,于病变染色边缘活检行病理学检查。结果 60例患者胃黏膜活检醋酸-靛胭脂染色后,高级别上皮内瘤变及低级别上皮内瘤变检出率分别为25.00%和31.67%,分别高于常规胃镜检查病灶边缘活检病例(13.33%和18.33%),差异有显著性(P<0.05)。78例行胃镜下醋酸-靛胭脂染色患者,高级别上皮内瘤变及低级别上皮内瘤变检出率分别为24.36%和30.77%,分别高于常规胃镜检查病灶边缘活检病例(11.54%和16.67%),差异有显著性(P<0.05)。结论醋酸联合靛胭脂色素内镜能够指导内镜下胃黏膜病变的活检,对于高级别上皮内瘤变及低级别上皮内瘤变的检出要优于常规胃镜检查。Objective To investigate the clinical value of chromoendoscopy in the detection of upper gastric diseases .Methods Two hundred and sixteen patients underwent endoscopy , with gastric mucosal of suspicious lesions biopsies. After acetic acid joint indigo carmine dye-ing , the edge of suspected upper gastric diseases in 60 patients was identified , with gastric mu-cosal of biopsies .In addition , 176 patients were divided into two groups , 78 patients under-went endoscopy with gastric mucosal of biopsies , the other 78 patients underwent acetic acid joint indigo carmine dyeing with gastric mucosal of biopsies .Results In 60 cases of gastric suspicious lesions , after acetic acid joint indigo carmine dyeing , the detection rate of high-grade and low-grade intraepithelial neoplasia were 25.00%and 31.67%, respectively , greater than biopsies （13.33%and 18.93%, P〈0.05 ）;24.36% of high-grade intraepithelial neo-plasia and 30.77%of low-grade intraepithelial neoplasia were found in 78 patients with acetic acid joint indigo carmine dyeing , greater than 11.54% and 16.67% in the other 78 patients with endoscopy only （ P〈0.05 ） .Conclusion Chromoendoscopy can guide endoscopic biopsy of gastric mucosal lesions , and improve the detection rate of high-grade and low-grade intraepi-thelial neoplasia .
