颅脑外伤所致精神异常的CT诊断及临床意义

摘　　要：目的探讨颅脑外伤所致精神障碍患者的CT诊断及临床意。方法回顾性分析2010年9月至2012年9月在佛山市第三人民医院就诊的86例颅脑外伤所致精神病患者的临床资料,分析颅脑CT检查结果以及病变部位。结果脑外伤所致精神病患者其脑部CT出现异常的机率86%,脑组织病变类型以脑萎缩、脑软化、脑积水、硬膜下积液、脑水肿较多见,病变部位主要以颞叶、额叶、基底节、枕(顶)叶、脑皮质、脑髓质或多部位病变常见。结论各脑叶损伤部位与精神障碍的出现密切相关,因此对颅脑外伤患者进行必要CT检查,为临床医生及时积极的综合治疗伴发的精神障碍,改善颅受损的恢复。Objective To investigate the clinical significance of CT and diagnosis of patients with mental disorders caused by brain trauma. Methods Retrospective analysis of clinical data of the Third People's Hospital of Foshan from 2010 September to 2012 September, 86 patients with psychiatric disorders were caused by brain trauma,brain CT examination results and analysis of the lesion were studied as well. Results In patients with psychiatric injury, the brain CT abnormal rate was 86%, brain tissue pathological type with cerebral atrophy,cerebromalacia, hydrocephalus, subdural effusion and brain edema were more common, the lesion was mainly in temporal lobe, frontal lobe, basal ganglia, pillow(top) lesions in cerebral cortex and brain matter leaves, or multiple sites. Conclusion The lobes of the brain damage and mental disorders are closely related, so CT examination is necessary for patients with traumatic brain injury, it helps clinicians give comprehensive treatment to patients complicated with mental disorder recover from brain damage.
