淋巴细胞免疫治疗复发性流产的效果分析

摘　　要：为了探讨淋巴细胞主动免疫治疗不明原因复发性流产患者的疗效,观察封闭抗体对妊娠结局的意义。本研究将门诊筛查确诊为封闭抗体缺乏所致的复发性流产患者120例,随机分为两组。研究组60例,采用淋巴细胞主动免疫治疗,对照组60例,采用传统保胎治疗,比较两组妊娠结局。研究组淋巴细胞主动免疫后复查封闭抗体,比较封闭抗体阳性组及阴性组妊娠结局。结果显示研究组60例中分娩37例,妊娠中期15例,流产8例,妊娠成功率86.7%。对照组60例,已分娩19例,妊娠中期12例,流产29例,妊娠成功率51.6%,两组相比有显著性差异。免疫治疗后,封闭抗体阳性组妊娠成功率88.6%,阴性妊娠成功率56.25%。两组相比差异有统计学意义。研究结果提示淋巴细胞主动免疫能提高封闭抗体缺乏所致复发性流产患者的再次妊娠成功率。To investigate the efficiency of active immunotherapy on recurrent spontaneous abortion （RSA） and to observe the significance of blocking antibodies in the subsequent pregnancy, One hundred twenty patients with RSA who lacked blocking antibodies were divided into two groups. 60 patients were subjected to active lymphocyte immunotherapy （the immunotherapy group）, the another 60 patients were received traditional conservative therapy （control group）, the difference in the outcome of pregnancy was compared between the tow groups. The results showed that, the rate of successful pregnancy in the group of immunotherapy was 86.7% （37 cases had delivered, 15 cases were in the second trimester of pregnancy, 6 cases aborted）, whereas that in the control group was 51.6% （19 cases delivered, 12 cases were in the second trimester, and 29 cases aborted, P〈0.05）. In the immunotherapy group, the success of pregnancy was highly related to the positive reverse of the blocking antibodies. Among those who tested positive for the blocking antibodies, the success rate of pregnancy attained 88.6%, while those who remained negative for the blocking antibodies, the successive rate was 56.25% （P〈0.05）. Taken together, these findings indicate that active immunization with lymphocyte can improve the rate of successful pregnancy for RSA patients due to deficiency of blocking antibodies.
