摘  要:目的评价本院自制剂硅霜I号配合糠酸莫米松乳膏治疗特应性皮炎的临床疗效和皮肤屏障修复作用。方法全部患者皮损处外用糠酸莫米松乳膏,另外治疗组外用硅霜I号,对照组外用橄榄油,疗程均为2周,于用药前、治疗第3d和14d时进行随访,评价靶皮损的皮肤干燥度、研究者总体评估(IGA)及主观症状,同时对靶皮损的皮肤屏障功能指标经皮水分丢失(TEWL)、角质层含水量和p H值进行测量。结果治疗第3d和14d时治疗组和对照组靶皮损皮肤干燥程度、IGA均较治疗前明显降低(P均<0.01),同时靶皮损TEWL和p H值明显下降,但其角质层含水量升高(P<0.01)。即治疗组的疗效和皮肤屏障改善程度优于对照组。两组不良反应轻微,呈一过性。结论本院自制剂硅霜I号配合糠酸莫米松乳膏治疗特应性皮炎的临床疗效良好,并可有效改善皮肤屏障功能,且安全性高。Objective To investigate the clinical efficacy and skin barrier recovery of Guishuangyihao cream in combi- nation with Mometasone furoate Cream for atopic dermatitis. Methods The patients were randomized into two groups. Lesions treated with topical 0. 1% Mometasone furoate cream plus Guishuangyihao cream served as treatment group while in control group, Guishuangyihao cream was replaced with olive oil. All patients were treated for 2 weeks and efficacy was evaluated on days 0, 3, 14. Assessments included skin dryness, investigator~ global assessment ( IGA), transepidermal water loss(TEWL) , stratum corneum(SC) hydration and pH values. Results In both groups, skin dryness, IGA,TEWL and pH declined significantly on days 3 and 14 compared to pretreatment(P 〈 0.01 ), while straum corneum (SC) hydration elevated ( P 〈 0.01 ). The improvement of IGA and skin barrier function in Guisbuangyihao cream was more dramatic than that in olive oil. Adverse reactions were mild and transient in both groups. Conclusion In addition to the improve- ment of permeability barrier, the in-house prepared Guishuangyihao cream combined with Mometasone fu- roate cream is also effective and safe for atopic dermatitis.
